3. ANN C8/3 is two alarms in one- a dump armed and a dump actuated alarm For a load drop, the steam dump armed alarms after a 10% ramp, the steam dump actuated alarms after a 9.5°F Tave-ref mismatch.

C8/3 may alarm for just the actuated portion of the alarm. But, Steam Dumps to Condenser may not actually open because they havent armed yet due to the load ramp/drop circuity not being satisfied. This was observed when a HDP tripped from 100% with auto rod motion failed- as is. Crews took manual control of SDC thinking it had failed.

The 10% Arm Circuity was a fraction of a point away from being satisfied.

  1. Operating Exam Comments Facility Comment #1 JPM A, Respond to Control Bank D Demanded Past ARO Steps, needed modification based on operator comment5 during prep week. The issue was with 3-ONOP-028 steps that were extraneous to the JPM. The JPM was shortened by removing the DCS response. AR# 2154662 was written to track PCR to 3/4-ONOP-028 to address the group step demand range on DCS as 0-230 steps. Comment Addressed Prep Week and incorporated into final submittal.

NRC Resolution:

Facility Comment #2 RO JPM A2, Review an ECO for the 3A Heater Drain Pump, needed modification based on JPM follow up questions. The JPM required a tag out of the 3A Heater Drain Pump. A critical step required the recirculation isolation valve to be closed prior to the suction. After discussing with the Facility Representative and reviewing 3-NOP-081, we have determined that closing 3-30-853, ISOLATION VALVE A HEATER DRAIN PUMP RECIRCULATING LINE, prior to closing the suction should NOT be included in the critical step.
