SUBJECT:
HUMAN FACTORS INVOLVEMENT AT OCONEE 1, 2, AND 3 Enclosed !Js
,a technical review of five events at Oconee involving compromises of containment to which human factors contributed. The review of these events concluded that Oconee had taken comprehensive efforts to eliminate or reduce human factors as a contributing cause of these and other types of future events.
While the technical review was:being prepared, an LER was received from Oconee in which the Emergency Closed Cooling Water System capability was lost because of human error (see Enclosure 2). Since one of the corrective actions to the five events involving compromise of containment was a review of directives and procedures related to independent verification of operational activities affecting safety-related equipment, the LER could be considered to be a measure of the success (or lack thereof) of the review. ROAB believed that the date of the ECCW event (830705) was too close in time to the Oconee review to have substantial significance as a measure of the success of the review,. NAS, therefore, decided to release the technical review as written.
