BNL VISIT TO DUKE POWER COMPANY, DECEMBER 3 - 4, 1990 This is to inform you that on December 3, 1990, Messrs. James C. Higgins and See-Meng Wong of the Brookhaven National Laboratory will visit the Duke Power Company offices in Charlotte, N.C., followed by a short visit to the Oconee Nuclear Station on December 4. The purpose of this visit will be to gather information related to human error root cause analysis and actions taken to prevent recurrence of important PRA-type human errors. Details of the type of information desired are included as an enclosure to this letter. Access to the protected area of the Oconee station should not be required. However, your support in providing the information requested would be appreciated.

ENCLOSURE PROJECED DIFORMATION AND PERSONNEL NEEDS FOR BNL VISIT TO DUKE POWER COMPANY, DECEM R 3-4,1990 Two people from BNL will visit Duke Power. They are:

James C. Higgins See-Meng Wong On December 3, 1990, they will go to Charlotte offces.

On December 4, 1990, they will go to the Oconee site.

The purpose of this visit is to collect information for the root cause project currently underway at BNL.

This project is Task 3 of FIN L-1170, which is sponsored by the Risk Applications Branch of NRR, NRC.

Earlier reports from related projects were published; namely, NUREG/CR-5319 and NUREO/CR-5527.

I.

Information Needs We desire to obtain information from Duke Power Company on all actions and activities taken to prevent important PRA-type human errors. This could include training, methods, procedures, administrative controls, etc.

It should also include corrective actions taken for actual event occurrences. Some examples of information to be reviewed are:

1)

Samples of Oconee PIR forms for reportable and non-reportable events.

2)

Root cause analysis methods to determine recurring events/problems, data trends associated with root causes and corrective actions taken to prevent problems.

3)

Lists of accident scenarios and descriptions, prioritized by PRA analysis, used in simulator training; results of crew response and follow-through on further training sessions.

4)

Upgraded EOP examples resulting from insights on crew response data of simulator training program and other PRA feedback,

5)

Data summaries of human-caused reactor scrams/ESF actuations (if available as separate data).

6)

List of important human errors in new Oconee PRA, their HEPs and description of use in simulator training to reduce HEP. Discussion of differences between new Oconee PRA and NSAC-60.

7)

Discussion with HPES knowledgeable person for Oconce.

8)

Discussion of training as It relates to risk significant errors,

9)

Administrative controls in place to preclude typical PRA human errors; e.g., valves left in Incorrect positions, miscalibration, failure to follow procedures during transient or accident.

II.

Anticipated Areas and Types of Personnel for Discussions

1)

Charlotte:

PRA, PIR system, Root Cause and Trending, Feedback of PRA and problem information to site, QA, Accident Strategies.

2)

Site: IPES, Training, Operations/Administrative Controls, Procedures.

We would prefer to have a short visit to the Charlotte offices and the Oconee site to discuss the information, and then bring back documents for review at BNL. We are willing to share our information (e.g., Root Cause Scheme) with Duke Power Company.
