APPENDIX A NOTICE OF VIOLATION Duke Power Company Docket No, 50-269 Oconee Unit 3 License No. DPR-55 As a result of the inspection conducted on May 10, 1981 through June 10,
- 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified.
10 CFR 50, Appendix B, Criterion V, as implemented by Duke Power Company Topical Report, Duke 1, Section 17.2.5, requires that activities affecting quality be accomplished in accordance with instructions, procedures or drawings. Procedure CP-209, Constructiohn Hydro, requires restoration of gagged relief valves following hydro testing.
Contrary to the above, on May 19, 1981, 3/4 inch relief valve, 3CS-124 was not operable, in that it was not restored to operable condition after hydro pursuant to the referenced procedure.
This caused a failure of four inch valve 3CS-5 and two inch valve 3CS-6 to remain tightly. closed, resulting in a loss of containment integrity for 5.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />.
There was no release to the environment.
This is a Severity Level V Violation (Supplement I.E.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be take.n to avoid further violations; and (5) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
