No. 2 Emergency Diesel Generator Fan Shaft Failure Fan Shaft Failure White violation of 10 CFR 50, Appendix B, Criterion III Design Control Criterion III, Design Control Exelon did not review the suitability of a new maintenance process for tensioning the cooling maintenance process for tensioning the cooling fan belt for the diesel generators.
This resulted in the shaft being more susceptible to fatigue and subsequent failure.
