Dear Mr. Hampton:
This inspection report documents the Nuclear Regulatory Commission's (NRC) response to
an event that occurred at Oconee Nuclear Station (ONS), Unit 3, on May 3, 1997, resulting in
degradation of the high pressure injection system during unit cooldown. The NRC chartered
an Augmented Inspection Team (AIT) to evaluate the event and the ONS' response to it.
The AIT completed its onsite inspection on May 9, 1997.
The AIT conducted an independent inspection of the event and circumstances surrounding it,
monitored the licensee's investigations into the incident, and reviewed other related
.
data/events. The AIT concluded that the event causes included: (1) lack of redundancy in
design of the letdown storage tank level instrumentation system; and, (2) loss of reference
leg inventory resulting in non-conservative and non-responsive level indication for both
letdown storage tank level instruments. These causes resulted in ONS being operated
outside its design basis during the period that the level instrumentation error caused letdown
storage tank levels to be lower than allowed. With a low letdown storage tank level, the
ability of the high pressure injection system to perform its safety function of mitigating a
design basis accident was not assured.
Also, during the Unit 3 cooldown on May 3, 1997, operators did not properly perform the
fundamental activities of monitoring and balancing reactor coolant inventory, and therefore,
they did not question the non-responsive letdown storage tank level indication, resulting in
the letdown storage tank being emptied of water to the point that degradation occurred on
two of the three high pressure injection pumps.
In addition to the causes discussed above, the AIT determined the root cause of the event
was inappropriate use of plant and industry operating experience to assure that plant design,
maintenance, and operation were focused on reliable operation of the High Pressure Injection
System/Components involved in this event.

The AIT found the licensee's response to the event after pump degradation occurred was
conservative, with the emergency plan being used to help staff and manage the high
pressure injection system recovery efforts in order to continue with the cooldown.
The AIT charter did not include the determination of compliance with NRC rules and
regulations or to recommend enforcement actions. These aspects will be addressed in a
subsequent inspection.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosures will be placed in the NRC Public Document Room (PDR).
