(NRC INSPECTION REPORT NOS. 50-269/96-16. 50-270/96-16,
AND 50-287/96-16)
Dear Mr. Hampton:
This refers to an inspection conducted on October 6 through November 16, 1996,
at the Oconee Nuclear Station. The inspection included a review of the
circumstances associated with several missing and incorrectly installed main
steam safety valve spindle nut cotter pins. The subject inspection report,
which identified the deficiencies as an apparent violation, was transmitted to
you by letter dated December 4, 1996. A predecisional enforcement conference
was conducted in the Region II office on December 18, 1996, to discuss the
apparent violation, the root causes, and your corrective actions to preclude
recurrence. Enclosed are NRC and licensee slides used during the presentation
and a list of attendees.
We recognize that you inspected the conditions of the cotter pins at the first
reasonable opportunity after you obtained information about a main steam
safety valve failure at another facility in May 1996. However, based on the
information developed during the inspections and the information your staff
provided during the conference, the NRC has determined that a violation of NRC
requirements occurred. The violation is cited in the enclosed Notice of
Violation (Notice) and the circumstances surrounding the violations are
described in detail in the subject inspection report. We are concerned that
the violation resulted from the failure of personnel to follow maintenance
procedures. Maintenance personnel failed to install cotter pins on two Unit 3
safety valves and improperly installed pins on four Unit 2 valves.
Poor
maintenance work practices could have resulted in the valves failing to reseat
which would complicate recovery actions from some plant transients.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. You should also address your actions
to improve the implementation of maintenance procedures on important plant
equipment. After reviewing your response to this Notice, including your
Oproposed corrective actions and the results of future inspections, the NRC
will determine whether further enforcement action is necessary to ensure
compliance with regulatory requirements.
