Submittal of the In-Service Inspection (ISI) Owner's Activity Report (OAR.
for the 2 0th Refueling Outage for Unit 2 and Supplement to the 18 Refueling Outage OAR for Unit 3 Enclosed in Attachment 1 is the ISI Owner's Activity Report covering ISI activities associated with the second period of the current ISI inspection interval for Unit 2. This report is submitted pursuant to the American Society of Mechanical Engineers (ASME)
Boiler and Pressure Vessel Code,Section XI, "Rules for the Inservice Inspection of Nuclear Power Plant Components," Article IWA-6200, "Requirements" (2001 Edition through 2003 Addenda) and ASME Code Case N-532-4 (Repair/Replacement Activity Documentation Requirements and In-Service Summary Report Preparation and Submission, section Xl, Division 1).
In addition, a supplement to the flaw evaluation for the Unit 3 18 ' Refueling Outage OAR concerning a closure head weld is being provided in Attachment 2. This indication was originally discovered during the 1 3 th Unit 3 Refueling Outage (3 rd ISI inspection interval) and was re-inspected satisfactorily in the 18' Unit 3 Refueling Outage in 2011 (4th ISI inspection interval). A summary of the evaluation performed for this closure head indication is provided in Attachment 2.

HCl Relevant Condition - Hanger identified as being misaligned. Condition was F-A F1.10 Hanger misaligned evaluated and it was determined that the hanger realigns due to thermal movement experienced during start-up. No additional actions required. (Ref IR 2405922)

This evaluation should have used the 2001 edition of the ASME Section Xl code since the evaluation was performed in the 4th SI Inspection Interval. This is a self-identified issue that was captured in the PBAPS Corrective Action Program (IR 1554519).
A code reconciliation was performed to ensure that the identified weld flaw was still acceptable in accordance with the ASME Section Xl 2001 edition. The reconciliation performed determined that the flaw evaluation met the requirements of the 2001 code with no changes required. The indication was originally discovered during the 13th Unit 3 Refueling Outage (3 rd ISI inspection interval) and was re-inspected satisfactorily in the 18't Unit 3 Refueling Outage in 2011 (4 th ISI inspection interval).
