ENCLOSURE 1 NOTICE OF VIOLATION Carolina Power and Light Company Docket No. 50-261 H. B. Robinson License No. DPR-23 During the NRC inspection conducted on February 11 -

March 10, 1989, a violation of NRC requirements was identified. In accordance with "General Statement of Policy and Procedure For NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violation is listed below:

Technical Specification 6.5.1.1.a requires that written procedures be implemented covering the applicable procedures recommended in Appendix A of Regulatory Guide (RG) 1.33, Rev. 2. Appendix A of RG 1.33, Section 9, part a.,

requires that maintenance which can affect performance of safety-related equipment should be performed in accordance with written procedures appropriate to the circumstances.

Administrative Procedure AP-006, Section 5.1, states that adherence to approved plant maintenance procedures shall be mandatory.

Contrary to the above, on February 15, 1989, plant maintenance procedure CM-111, revision 5 was not adhered to, in that, step 7.6.4 was initialed as being completed before the work was actually performed.

This step verified proper functioning of the Auxiliary Feedwater V2-14A and V2-14C valve operators, and was initialed before the operators had been completely reassembled.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Carolina Power and Light is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:

(1) admission or denial of the violation, (2) the reason for the violation if

admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
