SUMMARY

On Feburary 6, 2014 a Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway Exercise was conducted for the 10-mile Emergency Planning Zone (EPZ) around the Byron Station by the U.S. Department of Homeland Security (DHS)/Federal Emergency Management Agency (FEMA). The purpose of the exercise was to assess the capabilities of State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures.

The original November 20, 2013, date was postponed due to catastrophic weather events that occurred in central Illinois.

The most recent exercise at this site was conducted on July 27, 2011. The qualifying emergency preparedness exercise was conducted on November 15, 1983.

DHS/FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise. In the State of Illinois the risk counties of Ogle and Winnebago participated along with State Government.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.

Cooperation and teamwork on the part of all the participants were evident during this exercise.

This After Action (AAR)/Improvement Plan (IP) Report contains the evaluation of the biennial exercise and the evaluation of the following out-of-sequence interviews and activities: Illinois State Police (ISP) Dosimetry Control Officers Briefing, ISP Traffic and Access Control Point at the ISP District 1 Headquarters, Protective Actions for Schools (EV-2), Monitoring and Decontamination of Evacuees and Emergency Workers, Decontamination of Emergency Worker's Equipment, Public Registration and Congregate Care, Treatment and Transport of Contaminated Patients at Freeport Memorial Hospital and Byron Ambulance, Oregon Police Department Dosimetry Control Officer Briefing, Oregon Police Department Traffic and Access Control Points. Except where noted in this report, the State and local organizations demonstrated knowledge of and adequately implemented their emergency response plans and procedures.

SUMMARY

2.1 Exercise Purpose and Design The DHS/FEMA Region V Office evaluated the Byron Station REP Partial Participation Plume Exposure Pathway Exercise conducted on February 6, 2014, to assess the capabilities of local emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involvingthe Byron Station. The purpose of this report is to present the results and findings on the performance of the OROs during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities Exercise objectives and identified Capabilities/REP Criteria selected to be demonstrated are discussed in Appendix E "Exercise Plan."

The Exercise Planning Team (EPT) selected objectives that focus on evaluating emergency response procedures, identifying areas for improvement, and fostering collaboration between the various OROs and stakeholders. This exercise focused on the following objectives:

" ORO demonstration of effective Emergency Operations Management;

" ORO demonstration of effective Protective Action Decision Making;

" ORO demonstration of effective Protective Action Implementation;

" ORO demonstration of effective Emergency Notification and Public Information;

" ORO demonstration of effective Support Operations and Facilities.

2.3 Scenario Summary Appendix F "Scenario Details," contains a summary of the Exercise Scenario, a simulated sequence of events that was used as the basis for invoking emergency response actions by OROs during the Byron Station REP Partial Participation Plume Exposure Pathway Exercise on February 6, 2014.

Results of a technical review of the scenario, submitted on September 18, 2013, indicate that the scenario is adequate to support demonstration of DHS/FEMA requirements, as well as criteria selected by the OROs provided in the State's September 4, 2013, and revised January 22, 2014, 10

After Action Report/Improvement Plan Byron Station extent-of-play submissions. The Exercise Scenario and extent-of-play submissions were accepted by DHS/FEMA on October 4, 2013.

During the exercise, in addition to information and data provided through the Byron Station onsite scenario, controllers from the State of Illinois provided "inject messages" containing scenario events and/or relevant data to those persons or locations who would normally receive notification of such events. These inject messages were the method used for invoking additional specific response actions by OROs.

After Action Report/Improvement Plan Byron Station SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the February 6, 2014, Byron Station REP Partial 3 Participation Plume Exposure Pathway Exercise conducted to test the offsite emergency response capabilities of State and local governments in the 10-mile EPZ surrounding Byron Station.

Each jurisdiction and functional entity was evaluated based on its demonstration of exercise 3 criteria delineated in FEMA Radiological Emergency Preparedness: Exercise Evaluation Methodology, as published in the FEMA Radiological Emergency Preparedness Manual, dated June, 2013. Detailed information on the exercise criteria and the Extent-of-Play agreements used in this exercise are found in Appendix E "Exercise Plan" of this report.

Presented below are definitions of the terms used in this report relative to the criteria demonstration status:

  • M - Met: The jurisdiction or-functional entity performed all activities under the Demonstration Criterion to the level required in the Extent-of-Play Agreement, with no Deficiencies or ARCAs assessed under that criterion in the current exercise and no unresolved prior ARCAs.

- D - Deficiency: An observed or identified inadequacy of organizational performance in an exercise that could cause a finding that offsite emergency preparedness is not adequate to 3 provide reasonable assurance that appropriate protective measures can be taken in the event of a radiological emergency to protect the health and safety of the public living in the vicinity of a nuclear power plant.

- A - Area Requiring Corrective Action - An observed or identified inadequacy of organizational 3 performance in an exercise that is not considered, by itself, to adversely impact public health and safety. Listing of the demonstrated exercise criteria under which one or more ARCAs was/were assessed during the current exercise or ARCAs assessed during prior exercises remain unresolved. Included is a description of any ARCAs assessed during this exercise and the recommended corrective action to be demonstrated before or during the next biennial exercise.

- P - Plan Issue - An observed or identified inadequacy in the ORO's emergency plan or implementing procedures, rather than in the ORO's performance.

3.2 Summary Results of Exercise Evaluation The matrix presented in Table 3.1, on the following pages, presents the status of all exercise criteria from FEMA Radiological Emergency Preparedness: Exercise Evaluation Methodology, as published in the FEMA Radiological Emergency Preparedness Manual, dated June 2013, which were scheduled for demonstration during this exercise by all participating jurisdictions and functional entities. The criterion status box is blank if it Was not scheduled for demonstration.

This subsection provides information on the evaluation of each participating jurisdiction and functional entity in a jurisdiction-based, issues-only format.

The DHS/FEMA has developed a standardized system for numbering exercise issues. This system is used to achieve consistency in numbering exercise issues among DHS/FEMA Regions and site-specific exercise reports within each Region. It also is used to expedite tracking of exercise issues on a nationwide basis.

The identifying number of Deficiencies, ARCAs, and Plan Issues includes the following elements, with each element separated by a hyphen (-).

  • Plant Site Identifier - A two-digit number, corresponding to the Utility Billable Plant Site Code (09 for Byron Station).

Additionally, public inquiries rumors, and trend analysis were not fully addressed. The time between the end of the siren sounding (three minutes after activation) and the beginning of the broadcast of the emergency message to the public was excessive. For the initial Protective Action Decision (PAD) message, the elapsed time between the end of the siren sounding (1115 hours0.0129 days <br />0.31 hours <br />0.00184 weeks <br />4.242575e-4 months <br />) and the start of the message broadcast (1123 hours0.013 days <br />0.312 hours <br />0.00186 weeks <br />4.273015e-4 months <br />) was eight minutes. Specific rumor of erroneous areas being told to evacuate were not addressed in the Ogle County EOC and would not have been communicated to the public.

POSSIBLE CAUSE: The recording of the broadcast message took approximately six minutes each time. The sirens were sounded before the radio station was contacted to standby for a broadcast message. There was not:a Public Information Officer (PIO) available to record and analyze public inquiries, or to direct public information.

There was no Ogle County staff member assigned to reading, tracking or coordinating the dispensation of the recommended actions. A PIO knowledgeable in media and public relations, would have interacted with the State and the JIC in the preparation of news releases and coordination of responses to rumors, as outlined in IPRA Volume VI Chapter (2) page 57, Section K, "Public Information Considerations."

The NARS Form indicated that there was a radiological release and a protective action recommendation (PAR) toevacuate Sub-Areas 17,: 19, 23, 27, 39,. and 40. It also indicated that the wind was from 223 degrees at 6.2 miles per hour. The Ogle County staff and IEMA Radiological Advisor were also notified (via an inject message) that an accident had occurred on Illinois Route 72 and Illinois Highway 2 in Byron, IL located in Ogle County resulting in the blocking of the intersection. The evacuation route was going to be blocked for two hours. Using their hard copy evacuation route map with the affected Sub-Areas and downwind center line highlighted, the Ogle County Staff and IEMA Radiological Advisor identified the impediment location.

After a brief discussion, the Ogle County Staff and IEMA Radiological Advisor identified an intersection west of the impediment to re-route evacuation traffic. The re-route sent evacuees north on Colfax Street. The Ogle County Staff coordinated with the IEMA Radiological Advisor to ensure that the route would not expose evacuees to the radiological plume. It was determined that the new route would allow 25

There was one Deficiency identified during this exercise for Ogle County. There were no Deficiencies identified for the State of Illinois or Winnebago County.

Ogle County received a Deficiency finding under Protective Action Implementation Criterion I 3.d.2 - An implementation process involving identification and resolution of impediments to evacuation. 3 The Deficiency assessed was based on our findings that individual performances as well as the decision-making process used by Ogle County to implement protective action decisions failed to take into account radiological aspects, including the plume direction, when re-routing traffic after a controller injected traffic impediment was introduced. The Ogle County Representatives relayed the re-routing information (directions that had evacuees travel through the plume) to the Joint Information Center for release to the public. Evacuees were directed into and through the 3 centerline of the plume exposing them to radiological material, compromising their health and safety unnecessarily as alternate routes free from contamination and dose were available.

In accordance with 44 CFR 350.9(d), we thoroughly reviewed and discussed this issue with the Department of Homeland Security/Federal Emergency Management Agency Headquarters and the U.S. Nuclear Regulatory Commission.

The Deficiency was resolved during a re-demonstration exercise conducted with appropriate Ogle County personnel on April 2, 2014. 3 There were two Areas Requiring Corrective Action (ARCA) identified during this exercise for Ogle County. There were no ARCAs identified for State of Illinois or Winnebago County. There i was one ARCA from a previous exercise for Ogle County that was successfully redemonstrated.

The first ARCA for Ogle County was identified under Criterion L.a. 1 - OROs use effective procedures to alert, notify, and mobilize emergency personnel and activate facilities in a timely manner. The exercise criteria, contained in the FEMA Radiological Emergency Preparedness (REP) Program Manual (June 2013), represent a 3 functional translation of the planning standards and evaluation criteria of NUREG-0654/FEMA-REP- 1, Rev 1, "Criteria for the Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980. EVALUATION AREA 1 - EMERGENCY OPERATIONS MANAGEMENT Criterion 1.a.l: OROs use effective procedures to alert, notify and mobilize emergency personnel and activate facilities in a timely manner.

The State Emergency Operations Center (SEOC), which houses the IEMA Communications Center (Initial Warning Point), the State of Illinois Response Center (SIRC) and the Radiological Emergency Assessment Center (REAC), will provide SIMCELL support for county participation, however no State function will be evaluated since all State criteria have been successfully demonstrated in the last two years.

The Unified Area Command (UAC) will be represented via a SIMCELL which will be located in a breakout room in the SIRC. Radiological Assessment Field Team (RAFT) and the mobile lab will NOT be staffed but related tasks & functions will be represented via SIMCELL in REAC. IEMA personnel will be prepositioned in the local area and dispatched to the County EOCs. FEMA will not evaluate the TSC, JIC or EOF.

An actual phone call will be made to the FEMA duty officer advising them of the situation at Byron Station. Follow up calls to FEMA will be simulated.

The prepositioned State staff will begin exercise play no earlier than the Alert.

Criterion 1.b.1: Facilities are sufficient to support the emergency response.

This criterion will not be demonstrated during this exercise.

Criterion l.c.1: Key personnel with leadership roles for the ORO provide direction and control to that part of the overall response effort for which they are responsible.

This criterion will not be evaluated; however the State will participate to the level necessary to support county exercise play.

A DCO briefing/Traffic and Access Control (TAG) interview will be conducted by, and at, Illinois State Police District 1 on Tuesday, February 4 'h at 8:30 am (confirmed). During this event, the availability of dosimetry will be demonstrated. No more than 10% of ISP District 1 dosimetry kits will be inspected during the interview to confirm serial numbers.

EVALUATION AREA 2 - PROTECTIVE ACTION DECISION MAKING Criterion 2.a.l: OROs use a decision-making process, considering relevant factors and appropriate coordination, to ensure that an exposure control system, including the use of KI, is in place for emergency workers including provisions to authorize radiation exposure in excess of administrative limits or protective action guides.

This criterion will not be demonstrated during this exercise.

Criterion 2.b.l: Appropriate protective action recommendations are based on available information on plant conditions, field monitoring data and licensee and ORO dose projections, as well as knowledge of on-site and off-site environmental conditions.

Criterion 2.b.2: A decision-making process involving consideration of appropriate factors and necessary coordination is used to make protective action decisions (PADs) for the general public (including the recommendation for the use of KI, if ORO policy).

Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart. Appropriate record-keeping of the administration of KI for emergency workers is maintained.

A roster will be provided to demonstrate 24-hour staffing of the EOC. The evaluator will not be allowed to remove the roster from the EOC.

An evaluator will be positioned at the County Sheriffs Communications Center in Oregon (initial warning point) at the start of the exercise.

Criterion 1.b.l: Facilities are sufficient to support the emergency response.

This criterion will be demonstrated during this exercise. A new EOC is currently being developed in Ogle County and is expected to be functional by the exercise date.

School interviews will be conducted with the Byron CUSD #226 on Monday, February 3rd at 1:00 PM (confirmed) and the Oregon CUSD #220 on Monday, February 3rd at 3:00 pm (confirmed). At each, the Superintendent, a Principal, a Teacher, the transportation coordinator (DCO)/bus driver and the maintenance supervisor will be present at-the interview.

Criterion 3.d.1: Appropriate traffic and access control is established. Accurate instructions are provided to traffic and access control personnel.

Coordination between Ogle County agencies designated to provide traffic and access control will be exhibited in the County EOC. The initial instructional message to the public must include as a minimum the elements required by current REP guidance.

Ogle County will demonstrate the capability to provide both an alert signal and an instructional message to populated areas throughout the 10-mile plume pathway EPZ in a timely manner after their decision to activate the alert and notification system to implement protective action decisions (PAD).

This capability will be demonstrated through the simulated activation of the Byron Station EPZ Prompt Notification System as defined in IPRA-Byron. The Prompt Notification System (outdoor warning sirens) will not be activated during the exercise.

The ability to provide instructions to the public will be demonstrated through the simulated activation of the local radio station (WRHL). The local radio station will be contacted during the first, and possibly the second, alert and notification sequence. An evaluator and a controller will be positioned at the radio station during the exercise to monitor the initial message. If the initial message does not contain a PAR, the evaluator and controller will stay at the radio station until a message containing a PAR is received.

Winnebago County intends to activate the Emergency Operations Center (EOC) in Rockford, Illinois, to the appropriate level. The County will implement notification procedures as defined in IPRA-Byron. The EOC will be staffed with representatives from the County Board, ESDA Director or Coordinator, a Sheriffs Department representative, Highway Department representative and Regional Superintendent of Schools representative. If the scenario does not drive protective actions in Winnebago County, the County may decide to down-staff the EOC to a level sufficient to support Ogle County play and represent Winnebago County operational knowledge. A roster will be provided to demonstrate 24-hour County staffing of the EOC.

Evaluators will not be allowed to remove the roster from the EOC.

An evaluator will be positioned at the County Sheriff's Communications Center (initial warning point) at the start of the exercise.

Criterion 1.b.l: Facilities are sufficient to support the emergency response.

  • (0900) [EM task] - 1A RH pump breaker fails, indication in the control room is the indicating lights go dark and a ground alarm.
