NRC INTEGRATED INSPECTION REPORT NO. 50-261/97-12
Dear Mr. Keenan:
On November 22, 1997, the NRC completed an inspection at your H. B. Robinson
reactor facility. The enclosed report presents the results of that
inspection.
During the six-week period covered by this inspection period, your conduct of
activities at the H. B. Robinson facility was generally characterized by
safety-conscious operations, sound engineering and maintenance practices, and
careful radiological work controls. However, a violation of NRC requirements
was identified. The violation involves the failure to adequately control post
modification testing related to the Containment Air Recirculation Cooling
(CARC) System. This included an evaluation that was not conducted when a post
modification test acceptance criterion was not met and the failure to conduct
cooling capability test during post modification testing. This failure
contributed to the containment average air temperature design limit during
power operations being challenged; however, it did not adversely impact the
post-accident design function of the CARC System. Therefore, the violation
has been characterized as a Severity Level IV violation.
The violation is cited in the enclosed Notice of Violation (Notice), and the
circumstances surrounding the violation are described in detail in the
enclosed report. Please note that you are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements.
