NRC INTEGRATED INSPECTION REPORT NO. 50-261/97-09
Dear Mr. Keenan:
On August 30. 1997, the NRC completed an inspection at your H. B. Robinson
reactor facility. You were informed of the inspection findings in an exit
interview conducted on September 10, 1997. The enclosed report presents the
results of that inspection.
During the six-week period covered by this inspection period, your conduct of
activities at the H. B. Robinson facility was generally characterized by
safety-conscious operations, sound engineering and maintenance practices, and
careful radiological work controls. However, one violation of NRC
requirements related to the Limiting Safety System Setting (LSSS) for the Over
Power Delta Temperature (OPDT) reactor trip setpoint raised concern regarding
your control of calibration procedures for LSSS setpoints. This issue was
initially discussed in NRC Inspection Report No. 50-261/97-08. Due to an
inadequate calibration procedure, appropriate setpoint inputs were not
specified for the OPDT trip at power levels less than 100 percent. The NRC is
concerned that the resulting non-conservative error existed since 1979. The
NRC considered escalated enforcement action for the failure to control this
LSSS setpoint: however, our review of the violation concluded that the safety
significance of the error was low, and your review of other LSSS setpoints did
not identify any similar errors. In addition, the identification of the
condition during a power descention evolution by the Control Room Shift
Supervisor was an example of good attention to detail in the monitoring of
control room indications. Therefore, the violation has been characterized as
a Severity Level IV violation.
The violation is cited in the enclosed Notice of Violation (Notice), and the
circumstances surrounding the violation are described in detail in the
enclosed report. Please note that you are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements.
