PROPOSED IMPOSITION OF CIVIL PENALTY -
$50,000 This is to inform the Commission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 will be issued on or about October 23, 1992 to Carolina Power &
Light Company. This action is based on the failure to adequately implement foreign material exclusion control during a modification of the RHR system. Specifically, plastic material used as a welding purge dam was inadvertently introduced into the system, and blocked the safety injection (SI) pump recirculation line orifices.
As a consequence, the SI system performance over the range of design bases accidents was significantly degraded.
The base civil penalty was neither escalated nor mitigated for any of the factors in the Policy.
It should be noted that the licensee has not been specifically informed of the enforcement action. The schedule of issuance and notification is:
Mailing of Notice October 23, 1992 Telephone Notification of Licensee October 23, 1992 The State of South Carolina will be notified.
The licensee has thirty day s from the date of the Notice in which to respond.
Following NRC evaluation of the response, the civil penalty may be remitted,. mitigated, or imposed by order.
