SUBJECT: AUGMENTED INSPECTION TEAM REPORT REPORT NO. 50-261/89-20
This refers to the special, onsite inspection conducted by the NRC Augmented
Inspection Team (AIT) during the period August 28 - September 1, 1989.
The
inspection included a review of the events related to the identification of
inadequate net positive suction head for the Auxiliary Feedwater (AFW)
Pumps
at your H. B. Robinson facility.
At the conclusion of the inspection, the
findings were discussed with those members of your staff identified in the
enclosed inspection report.
The AIT concluded that- your lack of management attention to technical issues
and the resolution of those issues led to the late identification of this AFW
operability problem. This lack of management overview was compounded by the
following:
a lack of design basis information concerning the simultaneous
operation of three pumps;
the feeling that the
pumps were operable as
demonstrated by passing their surveillance tests; the non-prioritization of
your internal inspection findings; and the treating of the information as it
was developed at too low a level in your organization without management
involvement. Additionally, we are concerned with the apparent inability of
your implementing programs to promptly identify and elevate these types of
items to appropriate levels of management.
The enclosed copy of the AIT report identifies the areas examined during this
review. Within these areas, the inspection consisted of selective examinations
of procedures
and representative records,
interviews with personnel,
and
observation of activities in progress.
In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2,
Title 10, Code o.f Federal Regulations, a copy of this letter and its enclosure
will be placed in the NRC Public Document Room.
We appreciate your cooperation during our team inspection and subsequent
evaluation of this event.
