A special inspection conducted by the NRC Region II office on March 2-4, 1981 of activities at the H. B. Robinson facility indicated that inadequacies in your external radiation exposure control program resulted in each of three individuals receiving a total occupational dose to the whole body in excess of 3 rems. The inspection findings disclosed one overexposure in the first calendar quarter of 1980 and two additional overexposures in the third calendar quarter of 1980. On March 30, 1981, the Director of our Region II office met with your management to discuss our concerns about the radiation exposure control program at H. B. Robinson and the enforcement sanctions available to the NRC were also disclosed.
Our concern with regard to these overexposures is amplified because survey data was available, which should have indicated that there was potential for significant exposures to the head and gonads during entries into the steam generators. However, adequate evaluations of the hazards related to non uniform radiation fields were not performed which would have provided the necessary level *of personnel protection. The overexposures resulted from a lack of adequate attention to the exposure of the head and gonads during steam generator entries and inadequate personnel monitoring.
Once brought to their attention, your employees acted expeditiously to correct program inadequacies and provided measures to prevent the recurrence of dose limit violations. However, reevaluation of 1980 exposure records was not initiated until requested by the NRC. Nevertheless, failure to perform an adequate evaluation of. the radiation hazards associated with the non-uniform and complex radiation environment in your steam generators reveals a weakness in your external radiation exposure control program which resulted in three individuals receiving quarterly whole body occupational doses in excess of regulatory limits. Accordingly, we propose the imposition of civil penalties for the violations set forth in Sections IA and IB of Appendix A to this letter.

In view of the serious nature of the violations, involving unnecessary over exposures to radiation, the Interim Enforcement Criteria (45 FR 66754, October 7, 1980), are being applied for violations occurring after June 30, 1980.
In order to emphasize the importance of improving the performance of your radiation exposure control program and the necessity of complying with NRC requirements, we are proposing civil penalties of $40,000.
In view of this enforcement action for the two overexposures during the third quarter of 1980, enforcement action is not being taken for the overexposure in the first quarter of 1980.
You are required to respond to this letter and should follow the instructions specified in Appendix A when preparing your response. You should give particular attention to those actions designed to assure continuing compliance with NRC requirements. Your written reply to this letter, and the results of future inspections will be considered in determining whether further enforcement action is appropriate.
In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the enclosure will be placed in the NRC Public Document Room.
