APPENDIX A NOTICE OF VIOLATION Carolina Power and Light Company Docket No. 50-261 H. B. Robinson 2 License No. DPR-23 As a result of the inspection conducted on February 21 thru March 10, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violation was identified.
Technical Specification 6.8.1 requires that written procedures be established and implemented which satisfy the requirements of Section 5.1 of ANSI N18.7-1972. Section 5.1 of ANSI N18.7-1972 requires that procedures and performance testing be adequate to assure proper functioning of safety-related components on completion of modifications.
Contrary to the above, as of January 29, 1981, procedures and performance testing were inadequately established and implemented, such that the breaker overload trip settings on all four motor operated containment isolation valves for the Fire Suppression system were improperly set.
As a result, one valve did not close upon receiving a containment isolation signal.
This is a Severity Level IV Violation (Supplement I.D.3.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within twenty-five days of the date of this Notice, a written state ment or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
