OCT 3 0 1978 APPENDIX A NOTICE OF VIOLATION Carolina Power and Light Company License No. DPR-23 Based on the results of the NRC inspection conducted on October 2-6, 1978, it appears that certain of your activities were not conducted in full com pliance with NRC requirements as indicated below. These items have been categorized as described in our correspondence to you dated December 31, 1974.

A.

10 CFR 20.101(b) requires, in part, that the dose to the whole body, when added to the accumulated occupational dose to the whole body, shall not exceed 5(N-18) res where "N" equals the individual's age in years at his last birthday.

Contrary to the above, on July 25, 1978, Carolina Power and Light Company personnel discovered that a contract janitor had received exposure in excess of 5 rem, the permissible accumulated dose for his age of 19 years.

This is an infraction.

B.

10 CFR 71.5(b) requires that the Licensee conform to the standards and requirements of the Department of Transportation for intrastate shipments of licensed material.

49 CFR 173.393(h) states there must be no significant removable radioactive surface contamination on the exterior of the package. 49 CFR 173.393(n)(9) requires, in part, that the shipper insure that contamination levels are within the allowable limits prior to each shipment of any package.

Contrary to the above, drums of radioactive waste readied for shipment on October 4, 1978, had not been surveyed for surface contamination.

Licensee representatives stated that similar drums had been routinely placed in plastic bags and shipped with no contamination surveys performed.

This is an infraction.
