APPENDIX A NOTICE OF VIOLATION Carolina Power and Light Company License No. DPR-23 Based on the results of the NRC inspection conducted on August 1-4, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in our correspondence to you dated December 31, 1974.
Technical Specification 6.8.1 requires that written procedures be estab lished, implemented and maintained that meet or exceed the requirements and recommendations of Section 5.1 of ANSI N18.7-1972. Section 5.1 of ANSI N18.7-1972 requires in part that policies shall assure that documents, including revisions or changes are distributed to and used by personnel performing the prescribed activity.
Contrary to the above, the following periodic test (PT) procedures were not distributed and/or used by plant personnel:
- a. PT-4O.0 (Inservice Inspection Quarterly Valve Test) was performed on July 28, 1978 using a revision zero procedure that was superceded by revision 1.1 on April 18, 1978.
- b. The control room working file contained only revision 1.3 of PT-2.7C (Safety Injection System Component Test) which was superceded by revision 1.4 on June 30, 1978.
This is an infraction.
