1. Perform scan surveys of the required surface area at the prescribed scan rates. If less than a 100% scan is required, use professional judgment to select the areas scan surveyed. Select areas with the highest probability to contain residual activity (i.e., areas of known radionuclide usage, floor traffic pathways, cracks and/or crevices not easily cleaned, countertop workspaces, storage locations, stained or water spotted areas, or areas with a known history of contamination).
2.

If elevated activity is detected during the scan surveys, then establish the extent (size) of the contaminated area(s) and mark the affected area(s) and quantify the activity by taking static measurements of the affected area(s). Notify the Project Manager or designee of the type, amount, and extent of any contamination.

3. Document all scan survey results on the applicable survey maps and data results sheets.

The established number of static measurement locations needed for the statistical evaluation of this survey unit is 14. However, due to statistical and geometrical considerations, the actual number of locations may vary. In this case, collect the actual locations provided on the survey map whether this number is greater than or less than 14

5. Locate and mark the required static measurements locations using the survey map(s). Survey maps with sufficient detail to measure and locate all of these locations have been provided.
6.

Collect all required static measurements and document the results on the associated data results sheets.

Additional static measurements may be taken in suspect areas at the discretion of the Project Manager or survey technician. However, these additional locations are not included in the analysis of the statistical sample set.

7.

Removable contamination measurements (swipes) shall be performed at the established measurement locations and counted on a planchet counter accounting for alpha/beta contamination.

8. Notify the Project Manager or designee if elevated activity is detected by any of the static measurements or applicable removable contamination measurements.
9.

Ensure that all package information is completed and signed prior to turning in this Survey Package to the RSO or designee for review.

Survey Package Location Code Description RBC-200-1 I Page 3 of 5 A unique location code shall be assigned to each individual survey location to ensure proper data management of the survey results. The following format shall be used to ensure consistency throughout the final status survey process:

BBB-RRRR-SS-M-LLL Where:

BBB

= Building or area code. This field represents the facility area. This will be the building number of an assigned area number. (3 characters)

RRRR

= Survey unit ID code. This field represents the survey unit i.e. 2-01, 2-02 etc. (up to 4 characters).

Building system survey units (i.e. ventilation systems, drain systems, or vacuum systems) are assigned unique 4-digit numbers that are consistent with the building section and floor. (up to 4 characters)

SS

= Structural surface code. This field represents the structural surface such as floor, wall, ceiling, etc. and the surface division code. For example, if a room has a very large floor area, it may be divided into two or more divisions and each section would be assigned unique codes such as FI, F2, and so on. Walls may be divided on a frequent basis. If only one surface area is identified for a particular survey area, then the default number will be "1" (e.g., WI, FI, etc.). (2 characters)

M

= Structural material code. This field represents the type of structural material on which a particular measurement is taken. This field is usually used to track and assign specific material natural background levels. If not material background value is to be assigned, the default character shall be "M." (I character)

LLL

= Numerical identification number. This field represents the survey point assigned numerical identifier.

The field "001" means survey point location number I. Numerical identifiers shall not be duplicated within the same survey unit. (3 characters)

1. Perform scan surveys of the required surface area at the prescribed scan rates. If less than a 100% scan is required, use professional judgment to select the areas scan surveyed. Select areas with the highest probability to contain residual activity (i.e., areas of known radionuclide usage, floor traffic pathways, cracks and/or crevices not easily cleaned, countertop workspaces, storage locations, stained or water spotted areas, or areas with a known history of contamination).
2.

If elevated activity is detected during the scan surveys, then establish the extent (size) of the contaminated area(s) and mark the affected area(s) and quantify the activity by taking static measurements of the affected area(s). Notify the Project Manager or designee of the type, amount, and extent of any contamination.

3.

Document all scan survey results on the applicable survey maps and data results sheets.

4.

The established number of static measurement locations needed for the statistical evaluation of this survey unit is 14. However, due to statistical and geometrical considerations, the actual number of locations may vary. In this case, collect the actual locations provided on the survey map whether this number is greater than or less than 14

5.

Locate and mark the required static measurements locations using the survey map(s). Survey maps with sufficient detail to measure and locate all of these locations have been provided.

6.

Collect all required static measurements and document the results on the associated data results sheets.

Additional static measurements may be taken in suspect areas at the discretion of the Project Manager or survey technician. However, these additional locations are not included in the analysis of the statistical sample set.

7.

Removable contamination measurements (swipes) shall be performed at the established measurement locations and counted on a planchet counter accounting for alpha/beta contamination.

8. Notify the Project Manager or designee if elevated activity is detected by any of the static measurements or applicable removable contamination measurements.
9.

Ensure that all package information is completed and signed prior to turning in this Survey Package to the RSO or designee for review.

Perform scan surveys of the required surface area at the prescribed scan rates. If less than a 100% scan is required, use professional judgment to select the areas scan surveyed. Select areas with the highest probability to contain residual activity (i.e., areas of known radionuclide usage, floor traffic pathways, cracks and/or crevices not easily cleaned, countertop workspaces, storage locations, stained or water spotted areas, or areas with a known history of contamination).

2.

If elevated activity is detected during the scan surveys, then establish the extent (size) of the contaminated area(s) and mark the affected area(s) and quantify the activity by taking static measurements of the affected area(s). Notify the Project Manager or designee of the type, amount, and extent of any contamination.

3.

Document all scan survey results on the applicable survey maps and data results sheets.

4.

The established number of static measurement locations needed for the statistical evaluation of this survey unit is 14. However, due to statistical and geometrical considerations, the actual number of locations may vary. In this case, collect the actual locations provided on the survey map whether this number is greater than or less than 14

5.

Locate and mark the required static measurements locations using the survey map(s). Survey maps with sufficient detail to measure and locate all of these locations have been provided.

6.

Collect all required static measurements and document the results on the associated data results sheets.

Additional static measurements may be taken in suspect areas at the discretion of the Project Manager or survey technician. However, these additional locations are not included in the analysis of the statistical sample set.

7.

Removable contamination measurements (swipes) shall be performed at the established measurement locations and counted on a planchet counter accounting for alpha/beta contamination.

8.

Notify the Project Manager or designee if elevated activity is detected by any of the static measurements or applicable removable contamination measurements.

9.

Ensure that all package information is completed and signed prior to turning in this Survey Package to the RSO or designee for review.

1. Perform scan surveys of the required surface area at the prescribed scan rates. If less than a 100% scan is required, use professional judgment to select the areas scan surveyed. Select areas with the highest probability to contain residual activity (i.e., areas of known radionuclide usage, floor traffic pathways, cracks and/or crevices not easily cleaned, countertop workspaces, storage locations, stained or water spotted areas, or areas with a known history of contamination).
2.

If elevated activity is detected during the scan surveys, then establish the extent (size) of the contaminated area(s) and mark the affected area(s) and quantify the activity by taking static measurements of the affected area(s). Notify the Project Manager or designee of the type, amount, and extent of any contamination.

3.

Document all scan survey results on the applicable survey maps and data results sheets.

4.

The established number of static measurement locations needed for the statistical evaluation of this survey unit is 14. However, due to statistical and geometrical considerations, the actual number of locations may vary. In this case, collect the actual locations provided on the survey map whether this number is greater than or less than 14

5.

Locate and mark the required static measurements locations using the survey map(s). Survey maps with sufficient detail to measure and locate all of these locations have been provided.

6.

Collect all required static measurements and document the results on the associated data results sheets.

Additional static measurements may be taken in suspect areas at the discretion of the Project Manager or survey technician. However, these additional locations are not included in the analysis of the statistical sample set.

7.

Removable contamination measurements (swipes) shall be performed at the established measurement locations and counted on a planchet counter accounting for alpha/beta contamination.

8.

Notify the Project Manager or designee if elevated activity is detected by any of the static measurements or applicable removable contamination measurements.

9.

Ensure that all package information is completed and signed prior to turning in this Survey Package to the RSO or designee for review.

= Structural surface code. This field represents the structural surface such as floor, wall, ceiling, etc. and the surface division code. For example, if a room has a very large floor area, it may be divided into two or more divisions and each section would be assigned unique codes such as Fl, F2, and so on. Walls may be divided on a frequent basis. If only one surface area is identified for a particular survey area, then the default number will be "I" (e.g., WI, Fl, etc.). (2 characters)

= Structural material code. This field represents the type of structural material on which a particular measurement is taken. This field is usually used to track and assign specific material natural background levels. If not material background value is to be assigned, the default character shall be "M." (I character)

= Numerical identification number. This field represents the survey point assigned numerical identifier.

Survey Package General Survey Instructions: RBN-001-1 I Pape 2 of S I. Perform scan surveys of the required surface area at the prescribed scan rates. If less than a 100% scan is required, use professional judgment to select the areas scan surveyed. Select areas with the highest probability to contain residual activity (i.e., areas of known radionuclide usage, floor traffic pathways, cracks and/or crevices not easily cleaned, countertop workspaces, storage locations, stained or water spotted areas, or areas with a known history of contamination).

2.

If elevated activity is detected during the scan surveys, then establish the extent (size) of the contaminated area(s) and mark the affected area(s) and quantify the activity by taking static measurements of the affected area(s). Notify the Project Manager or designee of the type, amount, and extent of any contamination.

3. Document all scan survey results on the applicable survey maps and data results sheets.

The established number of static measurement locations needed for the statistical evaluation of this survey unit is 14. However, due to statistical and geometrical considerations, the actual number of locations may vary. In this case, collect the actual locations provided on the survey map whether this number is greater than or less than 14

5.

Locate and mark the required static measurements locations using the survey map(s). Survey maps with sufficient detail to measure and locate all of these locations have been provided.

6.

Collect all required static measurements and document the results on the associated data results sheets.

Additional static measurements may be taken in suspect areas at the discretion of the Project Manager or survey technician. However, these additional locations are not included in the analysis of the statistical sample set.

7.

Removable contamination measurements (swipes) shall be performed at the established measurement locations and counted on a planchet counter accounting for alpha/beta contamination.

8. Notify the Project Manager or designee if elevated activity is detected by any of the static measurements or applicable removable contamination measurements.
9.

Ensure that all package information is completed and signed prior to turning in this Survey Package to the RSO or designee for review.

Survey Package Location Code Description RBN-001-1 I Page 3 of 5 A unique location code shall be assigned to each individual survey location to ensure proper data management of the survey results. The following format shall be used to ensure consistency throughout the final status survey process:

BBB

= Building or area code. This field represents the facility area. This will be the building number of an assigned area number. (3 characters)

RRRR = Survey unit ID code. This field represents the survey unit i.e. 2-01, 2-02 etc. (up to 4 characters).

Building system survey units (i.e. ventilation systems, drain systems, or vacuum systems) are assigned unique 4-digit numbers that are consistent with the building section and floor. (up to 4 characters)

SS

= Structural surface code. This field represents the structural surface such as floor, wall, ceiling, etc. and the surface division code. For example, if a room has a very large floor area, it may be divided into two or more divisions and each section would be assigned unique codes such as F 1, F2, and so on. Walls may be divided on a frequent basis. If only one surface area is identified for a particular survey area, then the default number will be "I" (e.g., WI, F I, etc.). (2 characters)

M

= Structural material code. This field represents the type of structural material on which a particular measurement is taken. This field is usually used to track and assign specific material natural background levels. If not material background value is to be assigned, the default character shall be "M." (I character)

LLL

= Numerical identification number. This field represents the survey point assigned numerical identifier.

The field "001" means survey point location number I. Numerical identifiers shall not be duplicated within the same survey unit. (3 characters)

1. Perform scan surveys of the required surface area at the prescribed scan rates. If less than a 100% scan is required, use professional judgment to select the areas scan surveyed. Select areas with the highest probability to contain residual activity (i.e., areas of known radionuclide usage, floor traffic pathways, cracks and/or crevices not easily cleaned, countertop workspaces, storage locations, stained or water spotted areas, or areas with a known history of contamination).
2.

If elevated activity is detected during the scan surveys, then establish the extent (size) of the contaminated area(s) and mark the affected area(s) and quantify the activity by taking static measurements of the affected area(s). Notify the Project Manager or designee of the type, amount, and extent of any contamination.

3.

Document all scan survey results on the applicable survey maps and data results sheets.

4.

The established number of static measurement locations needed for the statistical evaluation of this survey unit is 14. However, due to statistical and geometrical considerations, the actual number of locations may vary. In this case, collect the actual locations provided on the survey map whether this number is greater than or less than 14

5.

Locate and mark the required static measurements locations using the survey map(s). Survey maps with sufficient detail to measure and locate all of these locations have been provided.

6.

Collect all required static measurements and document the results on the associated data results sheets.

Additional static measurements may be taken in suspect areas at the discretion of the Project Manager or survey technician. However, these additional locations are not included in the analysis of the statistical sample set.

7.

Removable contamination measurements (swipes) shall be performed at the established measurement locations and counted on a planchet counter accounting for alpha/beta contamination.

8.

Perform scan surveys of the required surface area at the prescribed scan rates. If less than a 100% scan is required, use professional judgment to select the areas scan surveyed. Select areas with the highest probability to contain residual activity (i.e., areas of known radionuclide usage, floor traffic pathways, cracks and/or crevices not easily cleaned, countertop workspaces, storage locations, stained or water spotted areas, or areas with a known history of contamination).

2.

If elevated activity is detected during the scan surveys, then establish the extent (size) of the contaminated area(s) and mark the affected area(s) and quantify the activity by taking static measurements of the affected area(s). Notify the Project Manager or designee of the type, amount, and extent of any contamination.

Document all scan survey results on the applicable survey maps and data results sheets.

The established number of static measurement locations needed for the statistical evaluation of this survey unit is 14. However, due to statistical and geometrical considerations, the actual number of locations may vary. In this case, collect the actual locations provided on the survey map whether this number is greater than or less than 14

5.

Locate and mark the required static measurements locations using the survey map(s). Survey maps with sufficient detail to measure and locate all of these locations have been provided.

6.

Collect all required static measurements and document the results on the associated data results sheets.

Additional static measurements may be taken in suspect areas at the discretion of the Project Manager or survey technician. However, these additional locations are not included in the analysis of the statistical sample set.

7.

Removable contamination measurements (swipes) shall be performed at the established measurement locations and counted on a planchet counter accounting for alpha/beta contamination.

8.

Notify the Project Manager or designee if elevated activity is detected by any of the static measurements or applicable removable contamination measurements.

9.

Ensure that all package information is completed and signed prior to turning in this Survey Package to the RSO or designee for review.

Survey Package Location Code Description ADM-401 I Page 3 of 5 A unique location code shall be assigned to each individual survey location to ensure proper data management of the survey results. The following format shall be used to ensure consistency throughout the final status survey process:

= Building or area code. This field represents the facility area. This will be the building number of an assigned area number. (3 characters)

RRRR

= Survey unit ID code. This field represents the survey unit i.e. 2-01, 2-02 etc. (up to 4 characters).

Building system survey units (i.e. ventilation systems, drain systems, or vacuum systems) are assigned unique 4-digit numbers that are consistent with the building section and floor. (up to 4 characters)

SS

= Structural surface code. This field represents the structural surface such as floor, wall, ceiling, etc. and the surface division code. For example, if a room has a very large floor area, it may be divided into two or more divisions and each section would be assigned unique codes such as Fl, F2, and so on. Walls may be divided on a frequent basis. If only one surface area is identified for a particular survey area, then the default number will be "I" (e.g., WI, Fl, etc.). (2 characters)

M

= Structural material code. This field represents the type of structural material on which a particular measurement is taken. This field is usually used to track and assign specific material natural background levels. If not material background value is to be assigned, the default character shall be "M." (I character)

LLL

= Numerical identification number. This field represents the survey point assigned numerical identifier.

The field "001" means survey point location number I. Numerical identifiers shall not be duplicated within the same survey unit. (3 characters)
