I. INTRODUCTION During an outage of San Onofre Unit 1 in June of 1979, all three steam generator feedwater nozzle to reducer and reducer to feedwater pipe welds were non destructively examined. As a result of these examina tions, samples were taken of the welds and adjacent areas for metal urgical examination. The purpose of this report is to describe the re sults of this inspection and examination program and to address the corrective action taken, or to be taken, as a result of the conditions found.
II. FEEDWATER SYSTEM AS BUILT CONFIGURATION A. Configuration The feedwater piping for San Onofre Unit 1 is 12" at the discharge from each of the feedwater pumps until a common 18" header is formed. Three 10" lines branch off this common header and penetrate containment. These branch lines are then attached to 10" X 14" reducers which are subsequently welded to the feedwater inlet nozzles on each steam generator. Figure I shows the original configuration for the San Onofre Unit 1 nozzle to re ducer to piping.
B. Materials The materials for the components are as follows:
- 1. Steam Generator Feedwater Inlet Nozzles - ASTM A 105 GR II.
- 2. 14" X 10" Reducers (Sched. 60 both ends) - ASTM A 234 WPB.
C. Welding Procedures All the field welding for these lines was done in accordance with Bechtel Procedure PI-AT-Lh-f Rev. 3, Dated 2/23/65. There were no backing rings or consumable inserts used. The root pass was done using gas tungsten arc welding with subsequent passes using shielded metal arc welding. There was no post weld heat treatment for the nozzle to reducer or reducer to piping welds for all three steam generators. All the nozzle to reducer welds and the B loop reducer to piping weld were done in the field. The A and C loop reducer to piping welds were shop welds.
D. Piping Analysis Stress analysis was performed on the SCE feedwater line configuration in an effort to determine the mechanism which could cause the observed cracking.
This analysis was broken into-two parts:
- 1) Structural analysis of the feedwater line including the effects of thermal, deadweight and pressure.
- 2) 2D finite element fatigue analysis of the feedwater nozzle/reducer configuration.
The structural analysis was performed using a 3D finite element model of the feedwater line with anchors included at the steam generator (SG) and containment penetration and the vertical and horizontal thermal growth of the SG applied at the feedwater nozzle. The WESTDYN7 computer code was used for the analysis. The geometry consisted of the feedwater nozzle, which is connected to a 14" to 10" reducer, followed by approximately four Dipe segments running to the containment penetration.
Sketches 1, 2 and 3 provide an isometric of each feedwater line layout and the location of the supports. Two thermal conditions were considered in the analysis.
The first with the SG at 547 0F and the feedwater line at 450'F representing normal operation. The second with the SG at 547 0F and the feedwater line cold representing the hot shutdown condition. The analyses results show a maximum thermal stress of approximately 8 ksi at the SG nozzle to reducer junction. The maximum deadweight and pressure stress were approximately 1 ksi and 6 ksi respectively. All of these stresses are well below code allowable values. The dead, thermal, and pressure stresses are indicated on the isometric sketches for each line.
The second analysis performed was a detailed 2D finite element fatigue analysis of the most severe thermal transient, which occurred during hot shutdown, in the region of the feedwater nozzle to reducer junction.The analysis used the WECAN computer code and the rules of ASME Section III, NB-3200.
The model used constant strain quadrilateral elements with a minimum of 8 nodes through the wall and ran from the SG shell to 15" beyond the nozzle reducer weld. The transient analyzed consisted of a ramp change in temperature from 547 0F to 60aF in 9 seconds followed by a period of constant 60aF operation, with a flow velocity of.38 ft/sec. This represents the injection of auxiliary feedwater into the feedwater nozzle/reducer which has been heated by the SG during the hot standby condition. When the auxiliary feedwater is terminated, a step change in temperature is assumed from 60aF to 547 0F. This represents the conservative assumption of a leaking check valve in the feedwater system, which allows water to flow from the SG to the feedwater line and assumes no mixing of auxiliary feedwater with water in the SG. The maximum peak stress range obtained from this transient was 53 ksi which is then multiplied by a factor of 1.7 to account for the girth butt weld at the nozzle safe end.
This peak stress range of 90 ksi yields an allowable 5500 cycles using the ASME Section III S/N curves. It should be noted that the transient analyzed is more severe than is expected in the plant and that analysis of the design transients given in the SG E-Spec has shown acceptable values of usage factors for the feedwater nozzle.
Correspondingly, analysis of the nozzle/reducer junction will have an acceptable value of usage factor since the thermal transient stresses are lower at this junction than in the nozzle.

' N-DESTRUCTIVE EXAMINATIONS A. Radiographic Examination A radiographic examination, using double wall technique, was performed on A, B, and C generator feedwater piping welds with the following results:

B.
Results of Radiographic Examinations Based on the results of these examinationasit was determined that all nozzle to reducer welds and the B reducer to pipe weld would be repaired.
The repair consisted of removal of each reducer and replacement with new reducers as further discussed in Section V of this report. In order to provide material for detailed evaluations of the indications revealed by radiography, each reducer also included the entire nozzle to reducer weld and approximately 1" to 1" of the nozzle base metal. Each reducer section thus removed was then radiographed using single wall technique for the purpose of locating and identifying specimens to be taken from these reducers for detailed metallurgical examination. The results of these radiographic examinations confirmed the results from the double wall radiographs.
C. Liquid Penetrant Examination And Results Following removal of the reducer sections, the I.D. bore of each nozzle was liquid penetrant examined with the following results:
A. A, B, and C nozzle bores exhibited significant amounts of pitting 3/16" maximum diameter by.030" deep in the lower 1200 quadrant.

"C" nozzle exhibited seven linear indications in the lower half of the nozzle. Six of these indications ranged in length from.9" to 2". One indication measured 8" in length.
IV. METALLURGICAL EVALUATIONS The reducer, reducer to nozzle weld, and a one inch portion of the feed water nozzle of steam generator B were examined. Single wall radiography had shown a series of indications at the weld running approximately 20 inches in the lower quadrant. A visual examination confirmed the presence of circumferential cracks in the affected area. In addition, the inside wall of the reducer was heavily pitted and showed cracks or tears in the oxidized surface. A three inch section was taken perpendicular to the cracks for additional examinations.
Figure two is a macrograph of the section at 7X. The nozzle, which is at the left, has a thicker wall than the reducer. A series. of parrellel cracks extend from the root of the weld to the end of the 3" reducer section. These cracks are about 0.1 inches deep and rather short, approximately k inch long. In fact,their intersection with the oxidized surface gives rise to the tears mentioned above. Another series of cracks, fewer in number and shallower in depth, can be observed on the nozzle bore.
Figure 3, which shows the crack in the weld at 50X, gives further details.
The cracks are very wide, branching and oxidized. Figure 4 shows that a crack, well away from the weld in the reducer, has the same morphology. The fact that the cracks are wide, corroded, and blunted at the tips is evidence that they have existed for a long time. Their appearance and branching na ture are usually associated with stress assisted corrosion. The residual stresses from welding (the weld was not post-weld heat treated) contributed to the observed phenomenon.
One crack, away from the weld, was broken open and the fracture faces exam ined by electron microscopy. Striations were observed on isolated areas in dicating that cyclic loading had taken place. In addition, a red corrosion product, probably a hydrated iron oxide, was seen over most of the fracture.
This is interpreted as additional evidence that the crack existed for a long time.

Although the investigation of specimens from feed water line B and the other two lines is continuing, we can draw conclusions from the evidence gathered to date. The mechanism of crack formation is concluded to be slow growth by stress assisted corrosion with intervals of fatigue crack growth.
From the paucity of regions containing striations and the fact that the cracks are not isolated to a stress concentration point, stress assisted cor rosion is considered to be the predominant failure mode.
V. CORRECTIVE ACTION A. Reducer Replacement The three reducers have been removed and are being replaced with new re ducers from material ordered to the original material specification.
Prior to reducer replacement, the circularfpitting indications in the ID bores of each nozzle were substantially removed by light grinding using a flappeJ wheel.
All linear indications in A and B nozzle bores were entirely removed and verified as such by liquid penetrant examination.
In both A and B nozzles, the final nozzle thickness after grinding exceeded the nozzle minimum wall thickness of.580". In the case of nozzle C, five of the seven linear indications were removed by grinding to depths below the minimum wall thickness of.580".
As a result, the region of nozzle C encompassing these indications was rebuilp by welding such that the final thickness exceeds.580".
The buildup region was verified as being free of linear indications by magnetic particles examination.
Both welds for installing these new reducers are being made using a minimum of 2000 pre-heat. Additionally, the more highly restrained weld joining the reducer to the thicker section nozzle will be subjected to a post weld heat treatment to minimize the levels of residual stresses in these welds and the adjacent material of the reducers.
All three reducers have been fabricated utilizing weld buildup of ends to facilitate clean up machining. This process assures that the resulting fitup and alignment will result in a minimal amount of offset or misalign ment further reducing the amount of resulting residual stresses in these welds. These buildups will be examined by UT and RT.
All weld crowns will be fully ground to provide an ideal geometry for radiography or ultrasonics depending on final decisions for volumetric examinations.
The welding process planned for use will utilize a gas tungsten arc root pass(s) to minimize the amount of root reinforcement; all fill passes will be accomplished with shielded metal arc welding. The reducer welds will be examined by MT and RT and will be subjected to a final leak test after welding. The grinding and care to be exercised during fitup and welding will permit future volumetric examinations to be performed by either UT or RT.
B.
Leak Test In order to provide assurance of the integrity of the reducer weldments, a leak test will be performed prior to returning the unit to service.
This test will consist of visual examination of the welds for leakage following approximately four hours in hot shut-down conditions (maximum secondary temperature and pressure of approximately 535 0 F, 925 psi.).
C. Puture Non-Destructive Examinations During the next refueling outage, each of the nozzle to reducer and reducer to pipe end welds will be volumetrically examined. Each such weld will again be volumetrically examined at least one additional time in the remainder of the current ten year ISI interval.

CONCLUSIONS The metallurgical results indicate that the mechanism of crack formation is slow growth by stress corrosion with intervals of fatigue crack growth. The stress assisted corrosion is considered the predominant failure mode. The metallurgical results indicate that the cracks have existed for a long time with a slow propagation rate.
This slow propagation rate in conjunction with the fact that the deepest crack found was less than 100 mils into piping sections 0.6 and 0.7 inch thick leadsto the conclusion that the structural integrity of the feedwater system was not impaired to an unacceptable degree.
Based on the facts that:
(1) the metallurgical results indicate that the cracking is a slow growth process, (2) all indications in the areas examined are being removed by grinding or replaced with new material which is subsequently examined and determined to be free of indications, (3) the reducer to nozzle welds will be subjected to a post weld heat treatment to minimize the levels of residual stresses in these welds and adjacent material, (4) each of the nozzle to reducer and reducer to pipe end welds will be volumetrically examined during the next refueling outage, and (5) all welds in the feedwater system piping are examinedduring the ten year inservice inspection interval, it is concluded that the San Onofre Unit 1 can resume power operation without undue risk to public health and safety.
