ATTACHMENT A Challange to Unit 1 PORV CV-454 Sheet SONGS Unit 1 Monthly Operating Report
- 0 DESCRIPTION OF A CHALLENGE TO UNIT 1 RELIEF VALVE On April 11, 1991, at 1002, 120 VAC Vital Bus 1 transferred to its backup power source due to a short on charging pump cooling water flow indicator control, FIC-609B. The pressurizer "PORV OPEN" and "PRESSURIZER HIGH PRESSURE ALARMS" were received and a very short, momentary, opening of PORV CV-545 resulted in an immediate pressure drop followed by a gradual increase in the pressurizer relief tank temperature and pressure. The vital bus was subsequently transfered back to its normal power supply at 1458 on April 11, 1991 with no repeat of this event.
CV-546 remained isolated during the event due to previously documented leakage.
The reporting of a challenge to a relief valve is required by Section 6.9.1.10 of Technical Specification to Provisional Operating License No. DPR-13 for San Onofre Unit 1.
