PRELIMINARY NOTIFICATION OF SAFEGUARDS EVENT PNS-V-88-07A Date: Dec. 7, 1988 This preliminary notification constitutes EARLY notice of events of POSSIBLE national security or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by the Region V staff on this date.

SUBJECT:
POSSIBLE TAMPERING EVENTS - CLOSEOUT PNS-V-88-07, dated December 1, 1988, reported possible tampering events. The more prominent event occurred on November 26, 1988, when an actuation alarm was received from the reactor coolant pump fire protection deluge system at Unit 2. During the licensee's investigation it was determined that 13 persons had access to this vital area. As a result of interviews they identified a contract watchman who admitted to activating the fire protection deluge system. He denied any intent to cause damage to the plant. During the interview he identified a second watchman who was present. The second watchman when questioned as to why he did not furnish complete information in his first interview, stated he wanted to give the other watchman an opportunity to confess. Both watchmen have been terminated. Of the 13 identified to be in the vital area, 11 have had their access restored and two have been terminated. The FBI has declined to take any action in this matter.
In September 1988, the licensee recalled that hoses associated with a temporary air ejector radiation monitor were not correctly connected. The licensee has conducted further investigation into this matter and concluded that tampering was not the probable cause. This has been verified by an onsite inspection.
Units 2 and 3 are at full power. Unit 1 is down for refueling.
