PR TMINARY NOTIFICATION OF SAFEGUARDS EVENT PNS-V-88-07 Date: Dec. 1, 1988 This preliminary notification constitutes EARLY notice of events of POSSIBLE national security or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by the Region V staff on this date.

FACILITY: Southern California Edison Company Emergency Classification San Onofre Units 1, 2, and 3 Notification of Unusual Event Docket Nos.

50-206, 50-361 and 50-362 Alert Site Area Emergency.

SUBJECT:

POSSIBLE TAMPERING EVENTS At approximately 3:00 PM, on November 30, 1988, the Senior-Resident-was informed by the licensee of possible tampering events.

The more prominent event occurred at approximately 7:45 AM, on November 26, 1988, when an actuation alarm was received from the reactor coolant pump 2P002 fire protection deluge system at Unit 2. Operators had responded and found no cause for the actuation. Further, no water had been sprayed as the system was isolated in accordance with design. The licensee determined that physical force was necessary to actuate the system alarm. The licensee is currently in the process of investigating the event.

At about 3:00 PM, on November 30, 1988, a high level sump alarm was received for the Unit 3 diesel generator room. The licensee found water entering the sump from the service water system. Subsequent investigation determined.that a cleaning person had been unable to.completely secure associated valving after work activities. Operators found the valves were difficult to operate. The licensee concluded that no malicious act took place.

In September 1988, the licensee recalled that hoses associates with a temporary Air Ejector Radiation Monitor were not correctly connected. The workman recalled correctly installing the hoses on the same day. This incident was not further investigated by the licensee at the time.

The licensee has taken the following action:

The FBI San Diego has been notified, but no investigation has been initiated to date.

An enhanced walkdown in addition to the normal shift walkdown has been performed. The walkdown focused on potentially susceptible areas and found no irregularities.

The computer access log for the area has identified seven individuals with access when the deluge system actuation occurred. Protected Area access for these individuals has been suspended pending interviews and investigation.

A Corporate Security investigation has been initiated into these events.

Licensee has made security and operations personnel aware of the incidents and directed that they heighten surveillance of plant safety systems.

The site is currently experiencing labor unrest related to benefits with their local unions which have resulted already in two informational pickets, with another scheduled.

IAT was notified at 4:19 PM, November 30,:1988. Region V is closely monitoring licensee actions regarding these incidents. Residenti.nspectors:are-conducting independent walkdown-inspections of the plant, and have thus-far foundnothing unusual., Resident inspectors have also queried reactor operators to insure they are informed of the incidents and are exercising increased care in monitoring plant systems and activities.

Units 2 and 3 are at full power. Unit 1 is down for refueling.
