Gentlemen:

This refers to the inspection performed by Messrs. E. T. Baker, J. B. Jacobson,

P. Prescott, R. Wilson, E. Trottier and R. McIntyre of the Vendor Inspection

Branch on February 2-13 and March 2-6, 1987 at the San Onofre Nuclear Generating

Station, Units 1, 2, and 3. The inspection related to activities authorized

by NRC License Nos. DPR-13, NPF-10, and NPF-15. Our findings were discussed

with Mr. H. E. Morgan and others of your staff at the conclusion of the

inspection.

The purpose of this inspection was to review the implementation of procurement,

vendor interface, and 10 CFR Part 21 programs at San Onofre. Additionally, a

review of specific vendor related technical issues was conducted.

One finding concerning the procurement of a commercial grade circuit breaker

for a safety related application and one finding concerning the potential use

of,unqualified butt splice connectors were identified. The two findings will

be classified as Potential Enforcement Findings (50-206/87-02-01 and 50-361/

87-03-01) and will be referred to the NRC Region V office for their review and

appropriate action.

POTENTIAL ENFORCEMENT FINDINGS

As a result of the inspection conducted on February 2-13 and March 2-6, 1987,

and in accordance with the "General Statement of Policy and Procedures for

NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the following

Potential Enforcement Findings were identified.

A. Potential Enforcement Finding 50-206/87-02-01

Criterion III, Appendix B to 10 CFR Part 50 states in part, "Measures

shall be established to assure that applicable regulatory requirements

and the design basis, as defined in §50.2 and as specified in the

license application, for those structures, systems, and components, to

which this appendix applies are correctly translated into specifications,

drawings, procedures, and instructions."

Criterion V, Appendix B to 10 CFR Part 50 states in part, "Activities

affecting quality shall be prescribed by documented instructions,

procedures, or drawings, of a type appropriate to the circumstances

and shall be accomplished in accordance with these instructions,

procedures, or drawings."

Contrary to the above, SONGS Unit No. 1 Equipment Qualification (EQ)

Document Package No. M38777 allows the use of AMP PIDG butt splice

connector terminals in any Class 1E system in any plant location. These

connectors, Model No. 53549-1, are not similar in construction to those

tested, although the environmental qualification is based upon similarity.

Additionally, for the connectors which were tested no performance criteria

were established and-failures which occurred during testing were not

properly justified.

B. Potential Enforcement Finding 50-361/87-03-01

10 CFR Appendix B, Criterion XVI, states, in part, "Measures shall be

established to assure that applicable regulatory requirements and the

design basis.. .for those structures, systems, and components to which

this appendix applies are correctly translated into specifications,

drawings, procedures and instructions.. .Measures shall also be established

for the selection and review for suitability of materials, parts,

equipment and processes that are essential to the safety-related

functions of structures, systems and components."

Southern California Edison's material control procedure S0123-XI-2.1,

paragraph 6.4, "Level III-Verification Method," states that this level

of procurement is applicable to quality affecting items whose critical

characteristics are capable of 6eing verified after receipt by inspection

and/or testing.

Contrary to the above, on Maintenance Order 8504117800 SCE installed in a

safety-related function and operated SONGS Unit 2 for 20 days with a

circuit breaker which had been purchased as commercial grade and was

inadequately evaluated for its ability to function during a seismic event.

At the end of the 20 day period, SCE replaced the breaker with one

received from Brown Boveri with documentation that it was qualified in

accordance with IEEE-344. The breaker's function is to control activation

of one train of main steam isolation and one train of auxiliary feedwater.

SCE's operability determination wa$ based on an external examination of

the replacement breaker. This examination provided no assurance that

the configuration of the operating mechanism of the replacement breaker

was sufficiently identical to the breaker which was originally qualified

by testing and does not justify the conclusion that the replacement

breaker would function properly during a seismic event.
