Steam Generator Repairs at San Onofre 1 This refers to the telephone discussion on October 15, 1980, between Mr. J. M.
Curran of your staff and Mr. J. L. Crews of this office regarding the recent discovery of foreign material in the hot leg piping of the "A" Steam Generator at San Onofre 1.
With regard to the matters discussed, it is our understanding that you will take the following actions:
- 1. Immediately halt work inside the steam generator primary channel heads until you have completed the following:
- a.
Revise written procedures to include accountability controls of all tools, equipment, materials, and supplies that are used in the channel heads to Prevent inadvertent entry of such items into the reactor primary cooling system. These controls are to be in effect whenever expandable seals and their associated cover plates are not in place in the nozzle(s) of the reactor cooling system piping.
- b.
Revise written procedures to restrict materials used in the channel head area to prevent the use of materials having potential adverse effects on the reactor cooling system components (for example chloride bearing materials).
- c.
Revise written procedures to include instructions to require that the channel head area (including the nozzle)be inspected and confirmed to be free of all loose materials, equipment, and tools prior to removing the aluminum cover from the expandable seals.

- 2.
Prior to the resumption of power operations, submit to this office for review and acceptance a written report covering an analysis of the possible effects of an foreign material which has entered the primary system and not been retrieved.
The report shall cover all work on the steam generators during the current outage.
If our understanding of your planned actions as described above is not accurate, you should contact this office promptly.
