NRC INSPECTION REPORT NO. 50-206,361,362/92-23
This refers to the routine inspection conducted by Messrs. C. W. Caldwell, D.
L. Solorio and C. D. Townsend of this office on July 17 through August 26,
1992, of activities authorized by NRC License Nos. DPR-13, NPF-10 and NPF-15
and to the discussion of our findings held by the inspectors with Mr. H. E.
Morgan and other members of the Southern California Edison staff at the
conclusion of the inspection.
Areas examined during this inspection are described in the enclosed inspection
.report.
Within these areas, the inspection consisted of selective
examinations of procedures and representative records, interviews with
personnel, and observations by the inspectors.
Based on the results of this inspection, one of your activities appeared to be
in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). This violation involved weaknesses in your surveillance
program and in understanding the importance of piston position in the
operation of valve accumulators, which resulted in Unit 1 Safety Injection
valve HV852B degrading over a three month period to an inoperable condition
without timely identification and correction.
In addition, one non-cited
violation involving the misalignment of a Unit 2 saltwater cooling pump valve
is discussed in the enclosed report. Our conclusion that this should be a
non-cited violation reflects your identification and prompt corrective actions
taken to resolve this issue.
We are also concerned with weaknesses in your measuring and test equipment
program which could lead to equipment being out of calibration for long
periods of time. Further, we encourage additional effort to enhance the
interface between Station Technical, Maintenance, and Operations, as indicated
by the plant trip that occurred during surveillance activities in Unit 2. In
addition, more effort appears warranted to ensure timely and effective
assessment of emerging problems as evidenced by several recent problems that
have occurred, such as those involving the pressurizer instrument line
leakage, station battery cracking, and misalignment of accumulator pistons for
a Unit 1 feedwater isolation valve.

You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. In your
response, you should document the specific action taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790, of the NRC's "Rules of Practice," a copy of
this letter, the enclosures, and your response will be placed in the NRC
Public Document Room.
The response directed by this letter and the accompanying Notice are not
subject to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, Pub. L.96-511.
