Gentlemen:

This refers to the routine inspection conducted by Messrs. C. W. Caldwell,

D. L. Solorio and C. D. Townsend of this office on June 4, 1992 through

July 16, 1992, of activities authorized by NRC License Nos. DPR-13, NPF-10 and

NPF-15, and to the discussion of our findings held by the inspectors with

Mr. H. E. Morgan and other members of the Southern California Edison staff at

the conclusion of the inspection.

Areas examined during the inspection are identified in the enclosed inspection

.report.

Within these areas, the inspection consisted of selective

examinations of procedures and representative records, interviews with

personnel and observations by the inspector.

Based on the results of this inspection, one of your activities appeared to be

in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). This violation is an example of weakness in the interface

between Station Technical (STEC) and Operations in controlling plant equipment

configuration during performance of surveillance activities. Specifically,

Station Technical personnel did not obtain approval from the Senior Reactor

Operator (SRO) Operations Superintendent to perform an inservice test of a

Unit 2 auxiliary feedwater pump. As a result, the SRO was not involved to the

level required by both Operations and Engineering procedures.

A second instance of this weakness in the STEC/Operations interface was noted

in which a Unit 2 salt water cooling (SWC) pump became inoperable due to a

misaligned valve as detailed in licensee event report 2-92-009. In that case,

Operations personnel failed to recognize that an Engineering procedure did not

comply with station requirements to provide for an Operations sign-off and an

independent verification of equipment manipulation. Thus, proper

configuration control was not maintained.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response. In your

response, you should document the specific action taken and any additional

actions you plan to prevent recurrence. Also, please include your evaluation

of the adequacy of the interface between Station Technical and Operations to

.ensure

proper configuration control of equipment. After-reviewing your

response to this Notice, including your proposed corrective actions and the

results of future inspections, the NRC will determine whether further NRC

enforcement action is necessary to ensure compliance with NRC regulatory

requirements.

In accordance with 10 CFR 2.790, of the NRC's "Rules of Practice," a copy of

this letter, the enclosures, and your response will be placed in the NRC

Public Document Room.

The response directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
