Gentlemen:

This refers to the Management Meeting on November 29, 1989 during which we

discussed issues related to the failure of an Automatic Switch Company (ASCO)

solenoid on August 23, 1989 and recent operator performance problems. In

addition, you provided a status on enhancements to the Emergency Preparedness

and Root Cause Assessment Programs, engineering program improvements, the

Design Basis Documentation (DBD) review, and the activities of Nuclear

Oversight Organizations. The subjects discussed during the meeting are

described in the enclosed Meeting Report for activities authorized by NRC

License Nos.

DPR-13, NPF-10 and NPF-15.

A copy of the agenda for this

meeting (transmitted to you by our letter dated November 22, 1989) is

enclosed.

With regard to the failure of the ASCO solenoid valve for CV-304, as pre

viously discussed in NRC Inspection Report No. 50-206/89-31, we are concerned

that sufficient information was available to have prevented the failure of

CV-304 and that appropriate information inputs were not made to the Control

of Problem Equipment (COPE) list. The COPE program appears to have been the

feature of your administrative controls which could most likely have

prevented the use of this problem equipment.

Weaknesses in your corrective

action program also appear to have contributed to the failure of CV-304.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure

will be placed in the NRC Public Document Room.

to discuss them with you.
