Yuhas (NRC), dated January 18, 1985 Pursuant to References (1) and (2), this is to inform you of maintenance activities being performed on the Reactor Coolant System Loop B & C grab sample facility isolation valve SV-3302. On May 9, 1985, SV-3302 was returned to service following completion of repairs to correct deficiencies in valve closure. On May 15, 1985, SV-3302 failed to open during sampling operations.
Since then, we have continued our investigation and maintenance on SV-3302.
On June 1, 1985, our investigation determined the failure of SV-3302 to open was due to valve binding when pressure exists across the valve. It was noted that when the line between SV-3302 and the upstream isolation valves (CV-955, CV-956) was depressurized, SV-3302 fully opened. By using special procedures involving entry into the sphere enclosure building to manually depressurize the line, samples were obtained on June 7, 1985. However, on June 21, 1985, when SV-3302 was retested, it failed to open and close. Since SV-3302 was determined not capable of automatic containment isolation, it has been maintained closed and the upstream inside containment isolation valves CV-955 and CV-956 were closed and deenergized. As a result, SV-3302 is inoperable for PASS.
