NRC INSPECTION OF SAN ONOFRE UNIT 1

This refers to the inspection conducted by Mr. C. A. Clark of this office on

January 9-20, 1989, of activities authorized by NRC License No. DPR-13, and to

the discussion of our findings held by Mr. Clark with members of your staff at

the conclusion of the inspection.

Areas examined during this inspection are described in the enclosed inspection

report. Within these areas, the inspection consisted of selective

examinations of procedures and representative records, interviews with

personnel, and observations by the inspector.

Based on the results of this inspection, it appears that one of your

activities was not conducted in full compliance with NRC requirements, as set

forth in the Notice of Violation, enclosed herewith as Appendix A. This

violation regards the failure to promptly document an apparent nonconforming

condition associated with the Unit 1 Residual Heat Removal system. We are

particularly concerned with this issue after considering several past problems

you have had in this area. Specifically, you reported in Unit 2 Licensee

Event Report 88-008 that a problem identified in 1983 with Component Cooling

Water (CCW) system leakage was not reported to the NRC due to a failure at

that time to initiate a Nonconformance Report (NCR). In NRC inspection report

50-362/87-03, another situation was described where pipe supports were

identified as missing from a system whose operability was required due to the

unit being in power operation, however the discrepancies were not evaluated

until several weeks later after the unit had shut down, apparently because an

NCR had not been written. Furthermore, during the NRC Safety System

Functional Inspection conducted in May-June, 1988, your staff found it

difficult to relate clearly what actions had been taken to assess the

significance of concerns raised regarding the CCW system because a one month

delay existed between the issuance of an engineering evaluation of the

concerns by your corporate engineering office, and the initiation and

disposition of those concerns by an NCR written by the plant staff.

This inspection also identified that your control of the use of ultrasonic

testing calibration blocks is weak. Your attention regarding this matter is

directed to paragraph 4 of the enclosed report.

Your response to the Notice of Violation is to be submitted in accordance with

the provisions of 10 CFR 2.201 as stated in Appendix A. In accordance' with 10

CFR 2.790(a), a copy of this letter and the enclosures will be placed in the

NRC Public Document Room.

The response directed by this letter and the accompanying notice is not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
