NRC INSPECTION REPORT 50-206/95-04; 50-361/95-04; 50-362/95-04 AND
This refers to the inspection conducted by inspectors identified in the
enclosed report of this office on March 12 through April 22, 1995. The
inspection included a review of activities authorized for your San Onofre
Nuclear Generating Station, Units 1, 2 and 3, facility. At the conclusion of
the inspection, the findings were discussed with those members of your staff
identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The purpose of the inspection was to determine
whether activities authorized by the license were conducted safely and in
accordance withNRCr.equirements.-.
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
The first violation is of concern because it includes two
examples of inadequate procedures which resulted in improper system
alignments. Since late 1993, there have been several potentially significant
system alignment errors that include instances of system operation either
without a discharge path, at excessive flow rates, or with no cooling water
flow, and of near operation without a suction flow path. These errors were
all related to inadequate or poorly implemented procedures.
This inspection report also discusses operators' lack of awareness of certain
control board indications and failures to understand Technical Specification
requirements for certain breaker alignments. In addition, weaknesses were
identified in the instructions given for breaker alignment and equipment
deficiencies were allowed to go uncorrected which caused unnecessary
distractions to operators during a reactor startup. These things indicate a
need for continued management attention to operations in the control room.
The second violation, dealing with corrective actions related to long standing
deficiencies in performance of Agastat relays used for timing loading of the
emergency diesel generators, is of concern because of Engineering's failure to
resolve the matter resulting in the repeated failures to meet Technical
Specification acceptance criteria. The Nuclear Oversight Division was

proactive in issuing a stop-work order to the nuclear fuel vendor (ABB/CE)
when a manufacturing process problem was not being adequately addressed.
During this period, two events occurred involving a loss of inventory from the
reactor coolant system. While neither event had serious consequences, the
potential compromise of the inventory control safety function is very serious
and deserves your attention to eliminate undue risks. Operations department
performance weaknesses were particularly evident in one of the events.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.
Your response may reference or
include previous docketed correspondence, if the correspondence adequately
addresses the required response. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of
this letter, its enclosures, and your response will be placed in the NRC
Public Document Room (PDR).
To the extent possible, your response should not
include any personal privacy, proprietary, or safeguards information so that
it can be placed in the PDR without redaction. However, if you find it
necessary to include such information, you should clearly indicate the
specific information that you desire not to be placed in the PDR, and provide
the legal basis to support your request for withholding the information from
the public.
The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
