APPENDIX A Southern California Edison Company P. 0. Box 800 Rosemead, California 91770 Docket No. 50-361 Construction Permit No. CPPR-97 Notice of Violation Based on the results of an NRC inspection conducted on August 13-31, 1979, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Construction Permit No. CPPR-97 as indicated below:
Appendix B of 10 CFR 50, Criterion V, states in part, that, "Activities affecting quality shall be prescribed by documented instructions.
and shall be accomplished in accordance with these instructions...
Appendix A to the PSAR commits to comply with the requirements of 10 CFR 50, Appendix B.
Contrary to the above, the requirement of construction specification CS-EO1, Revision 3, "Installation of Electrical Cables in Conduit and Duct Banks" was not followed when the calculations of the maximum expected tension for the pull were made. A cable pull (No. EP-151) through a conduit and duct bank was done in one con tinuous operation. The calculations for the conduit and duct bank were not added to determine the need for a tension limiting puller as required by the specification. The calculations were reviewed by.the NRC inspector on August 30, 1979 and found to be in error.
This is an infraction.
