BOARD NOTIFICATION OF SAN ONOFRE 2 CORE BARREL MISORIENTATION It would appear to be prudent to notify the San Onofre 2 and 3 Board of this occurrence at this time. As you know, we expect a low power decision from the Board in the near future. The core 'arrel misogientation probably occurfed because the reactor vessel as installed with a 180 misorientation, thereby necessitating a similar 180 rotation of the core barrel. In this instance, the core barrel was installed as if the vessel were not misori:ented, which was incorrect. Since the original vessel misorientation was a "mirror image" problem and a QA problem, and the core barrel misorientation can be considered a QA problem, the situation is reminiscent of the recent problems at Diablo Canyon. Probably because of this, Commissioner Bradford's office has expressed interest in this occurrence, and whether or not the Board will be notified.
Until the applicant files the 30-day report, and the Region completes its evaluation, it is difficult to state categorically that this occurrence has no safety significance. On this basis we have prepared the attached board notification.
