Attachment to Form NIS-1 McGuire 2 EOC 21 Steam Generator Tube Inspection Report Pursuant to ASME Section XI and McGuire Technical Specification 5.6.8 the following information is provided:

a. The scope of inspections performed on each SG Baseline inspection scope includedfull length data acquisition and bobbin coil data analysis on allfour (4) steam generatorsas follows.
1) 50% random sample of all tubes in allfour Steam Generators.
2) All tubes with previous indications,e.g., wear, DNT, PLP, etc.
3) All tubes surroundingplugged tubes.
4) Periphery tubes two rows deep in the hot leg and cold leg (outer perimeter and open lane).

Special interest inspection scope included data acquisition and array data analysis as follows:

1) Periphery tubes (5 tubes infrom periphery)from top of tubesheet to the first support in both hot leg (TSH to O1H) and cold leg (TSC to 01 C).
2) Bounding inspection two tubes deep for tubes plugged last inspectionfor loose parts.
3) All new DNT and new TWD reported duringEOC21.
4) All DNT like anomalies at the top of tubesheet in SG A.
5) All bobbin I-codes and PLP.
6) All confirmed ECTPLP callsfrom the previous inspection.

Plug inspection scope asfollows.:

1) Visual inspection of allplugs Secondary Side visual inspection scope as follows:
1) Top of tubesheet, five tube pitches deep from the periphery and tubefree lane in allfour Steam Generatorsfor loose parts.
2) Steam Drum inspection and seventh lateralsupport in the 2A SG.
b. Active degradation mechanisms found Tube wear at support structures andfrom loose parts was the only degradation mechanism found.
c. Non-destructive examination techniques utilized for each degradation mechanism

Bobbin was used to detect wear at support structures.Bobbin, array, and +Pointwere used to detect and characterize wearfrom loose objects.

d. Location, orientation (if linear), and measured sizes (if available) of service induced indications The complete listingfor service induced indications is referenced in attachment#1.
e. Number of tubes plugged during the inspection outage for each active degradation mechanism In the 2B SGfive tubes were plugged as a preventive measure due to a foreign object that could not be removed. In the 2C SG eight tubes were plugged due to wear and as a preventive measure associatedwith a foreign object that could not be removed.

g. The results of condition monitoring, including. the results of tube pulls and in-situ pressure testing An NDE maximum depth call of 49% TWor less is sufficient to demonstrate a minimum degraded tube burstpressure of 3AP at 0.95 probabilitywith 50%

confidencefor wear at support structures. This calculation considers a potential2% MUR power uprate at McGuire Unit 2 and associatedincrease in the operatingpressurefrom the previously used value of 3,900 psi to 4,125 psi.

This value is especially conservative as the uprate has not yet been performed.

The worst case support structure wear call observed duringthe McGuire 2EOC21 inspectionfor wear at structureswas 27% TW. Therefore, condition monitoringstructuralintegrity has been demonstratedfor support structure wear. Since the burstpressure equation treats though-wall tearingas coincidentwith burst, demonstrationof structuralintegrity also demonstrates leakage integrity at the lower differentialpressureof a postulatedsteam line break accident.

In addition to structuralsupport wear, there was one tube in SG 2C which exhibited two indicationsofforeign object wear. This tube (Row 109, Column

72) and those surroundingtheforeign object were plugged because the part could not be retrieved. Both indicationswere examined with a +Pointprobe to demonstrate condition monitoring acceptability. The EPRI Steam GeneratorIn Situ Pressure Test Guidelinesprovide screening criteriaforforeign object wear with a +Pointexam. The acceptance criterion is quoted below for foreign object wear:

"Forforeign object wear and cold leg thinning if VM < 2 volts and axial length

is <1 inch, condition monitoring is met for structuralintegrity and a prooftest is not necessary.

The +Pointsizing of theforeign object wear indicationsin SG 2C were 0.49 and 0.52 volts and were 0.23 and 0.38 inchesfor a combined length of 0.61 inches, thus easily meeting condition monitoring requirements.

All plugs were visually inspected. No anomalies ofplugs were found.

Secondary side inspections were performed on allfour SG's at the lower tubesheet. A number offoreign objects were identified in the tubesheet region.

Theseforeign objects were either removed, evaluated to be acceptable to leave in place, or surroundingtubes were preventively plugged.

A Steam Drum and 7th latticegrid visual inspection was performed in the 2A SG. Some expected erosion of the primary and secondaryseparatorswas noted.

No other issues were noted.

No in-situ testing was required orperformed. No tube pulls were performed.

h. The effective plugging percentage for all plugging in each SG The effective pluggingpercentagefor each of the McGuire Unit 2 Steam Generatorsis identicalto those shown in section 69 above.
