6.0 CONTAINMENT ISI PROGRAM There were no containment concrete inspections in this period. Therefore, provisions of 10CFR50.55a(b)(2)(viii)(E) through (b)(2)(viii)(G) do not apply.

However, provisions of 10CFR50.55a(b)(2)(ix)(A) do apply due to indications found during the inspection of moisture barrier in B1 R18.

(1) The entire length of moisture barrier at elev. 377' of the containment structure was examined by a direct examination method (VT-1) under WO #01419539-01 and it was found to have been separated at 4 different locations (IR #01411023) from the trough channel used for drainage collection. The separation was attributed to inadequate application of moisture barrier material during the previous installation.

(2) The moisture barrier does not provide any pressure boundary function other than being used as a protection for the liner plate against possible corrosion. The degraded moisture barrier was removed at the affected areas and the subsequently-exposed liner plate was then examined under WO #01572126. The examination revealed the liner plate uncoated surfaces contained pitting at several locations with a maximum metal loss in excess of 10%

at only two locations (IR #01417251). The liner plate condition at these areas was evaluated and the degradations were found to be acceptable by comparison to the acceptance criteria of 50% liner material loss based on an existing evaluation.

(3) The degraded conditions of the liner plate were found acceptable by evaluation without repair/replacement activities. The degraded moisture barrier was replaced with like for like at all the affected areas.

Subject:

R3 MOISTURE BARRIER DEGRADED AT 4 SEPARATE LOCATIONS Condition

Description:

During the scheduled inspection of the moisture barrier along the outer liner plate at elev. 377' of the U-1 containment during B1 R18, the condition of the moisture barrier was noted to have been degraded at 4 separate locations as follows:

1) Between R12 and R13, degraded area begins 12" to the right of the leak chase and continues for approximately 3'
2) Between R20 and R21 and directly below penetration P86, degraded area is approximately 12" long
3) Between R21 and R1, degraded area is approximately 6" long and is located one third way from R1
4) A degraded area approximately 12" long located directly below P74.

No operability concern exists. All degraded areas are in the form of slight separation of the moisture barrier top surface from the outer edge of the trough. No full separation has occurred and therefore, no moisture was able to reach the liner plate.

The recommended action is to fully restore the degraded areas prior to exiting 61 R18 in order to prevent further degradation of the affected areas.

Immediate actions taken:

1) Completed the visual inspection of the moisture barrier around the full circumference of the containment outer wall Section 3.0 Page 1 of 3
2) Walked down the affected area with the Williams Specialty Level III coating specialist.

Recommended Actions: Generate a WO to:

1) remove the existing degraded moisture barrier at the affected and aforementioned areas,
2) Remove protective coating on the newly exposed liner plate directly behind the affected moisture barrier,
3) Require Programs Engineering to perform a visual inspection of the exposed liner plate
4) Prepare the exposed liner plate areas and apply approved Level I coating on the affected areas
5) After the proper curing of the coating, install replacement moisture barrier in accordance with the design drawings at the affected areas.

Functional Basis:

The Moisture Barrier is functional but degraded.

LINER PLATE METAL LOSS MORE THAN 10%

Condition

Description:

During the performance of the exposed liner plate inspection where the degraded moisture barrier was removed at four separate locations for replacement (IR #1411023), the liner plate material loss due to inactive corrosion exceeded the allowable limit of 10 % per the ASME Section XI, Subsection IWE.

Based on the documented inspection results (WO #1572126-02), the liner plate behind the removed moisture barrier was observed to have had a film of rust due to corrosion that was determined not to be active. This necessitated pitting depth measurements at these areas. The surfaces of the liner plate at these locations were also not coated from the original construction.

In fact, liner plate behind the moisture barrier is not coated at all. The observed inactive corrosion is the result of introduction of moisture at the degraded moisture barrier locations since B1 R17 as no moisture barrier degradation was observed at that time.

The following pit depths were then obtained (the nominal liner plate thickness is 0.250" or 16/64"):

At area 1: 1/32" (>10 % metal loss, effective nominal wall thickness remaining: 14/64")

At area 2: 1/64" (<10% metal loss, effective nominal wall thickness remaining: 15/64")

At area 3: 1/64" (<10% metal loss, effective nominal wall thickness remaining: 15/64")

At area 4: 1/32" (>10% metal loss, effective nominal wall thickness remaining: 14/64").

(The above areas are defined in the work package WO #1572126-02).

Per the ASME code Section XI, Subsection IWE, liner plate metal loss of more than 10% shall be evaluated for acceptability or repair / replacement.

As a supplemental examination, Ultrasonic examination were performed and the following min.

liner plate thickness was obtained (UT report #2012-323 and 2012-327):

At area 1: 0.220" At area 2: 0.254" At area 3: 0.246" At area 4: 0.222".

There is no operability concern. The liner plate min. thickness may be 0.125" (or 8/64") per NDIT #BYR- 99-226. Therefore, the minimum wall thickness met the owner -established Section 3.0 Page 2 of 3

acceptance criteria. Furthermore, the remaining areas are not suspected of having liner plate corrosion since the inspection of the moisture barrier in B1 R18 did not identify any other moisture barrier degradation.

Therefore, the current condition of the liner plate is acceptable as-is until B1 R19 where liner plate will be coated in its entirety and, the liner plate is properly coated at the degraded locations. Even if the remaining areas were to develop degradation based on any hypothetical moisture barrier degradation during the next full Unit operating cycle, the rate of liner plate degradation will not exceed the available nominal margin as the liner plate had at most degraded by only 2/64" during one refueling outage. Additionally, the UT results had indicated the liner plate to be thicker than the nominal wall thickness and therefore, additional margin exists.

Immediate actions taken:

1) Identified there were no other degraded moisture barrier in U-1.
2) Removed the degraded moisture barrier and cleaned the exposed liner plate surfaces behind the removed moisture barrier.
3) Completed pitting depth measurements.
4) Completed supplemental UT examinations at the affected areas.
5) The affected liner plate is coated using an approved Level I coating.
6) New moisture barrier will be installed where the degraded moisture barrier was removed (per WO #1572126-01).

Recommended Actions:

1) An action to Engineering programs to complete an evaluation as the result of 10% metal loss condition as required by the ASME section XI, Subsection IWE (due 180 days).
2) An action to Engineering Programs to scope in the B1 R19 outage scope a WO to replace the entire moisture barrier and apply level I coating to the exposed liner plate (due 180 days).

Operable Basis: U-1 Containment is OPERABLE. There is no operability concern. The liner plate min. thickness may be 0.125" (or 8/64") per NDIT #BYR-99-226. Therefore, the minimum wall thickness met the owner-established acceptance criteria. Furthermore, the remaining areas are not suspected of having liner plat corrosion since the inspection of the moisture barrier in 131 R18 did not identify any other moisture barrier degradation. Therefore, the current condition of the liner plate is acceptable as-is until B1 R19 where liner plate will be coated in its entirety and, the liner plate is properly coated at the degraded locations.
