Location (Bldg, Elev, Room/Area):
Area 18 - SC, 175'-0" - N-E Quadrant Instructions for Completing Checklist This checklist may be used to document the results of the Area Walk-By near one or more SWEL items. The space below each of the following questions may be used to record the results of judgments and findings.
Additional space is provided at the end of this checklist for documenting other comments.
- 1. Does anchorage of equipment in the area appear to be free of potentially adverse seismic conditions (if visible without necessarily opening cabinets)?
Yes
- 2.
Does anchorage of equipment in the area appear to be free of significant degraded conditions?
Yes
- 3.
Based on a visual inspection from the floor, do the cable/conduit raceways and HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,
condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?
Yes
- 4.
Does it appear that the area is free of potentially adverse seismic spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?
Yes
- 5.
Does it appear that the area is free of potentially adverse seismic interactions that could cause flooding or spray in the area?
Yes
- 6.
Does it appear that the area is free of potentially adverse seismic interactions that could cause a fire in the area?
Yes D-71

- 7.
Does it appear that the area is free of potentially adverse seismic interactions Yes associated with housekeeping practices, storage of portable equipment, and temporary installations (e.g., scaffolding, lead shielding)?
Flammable storage cabinet and fire extinguisher not tied down well. Fire extinguisher has a very short hook. Cabinet is adjacent to trip sensitive equipment rack 017. Cabinet has a posted Material Area Storage Permit with a referenced seismic evaluation within the processes for storing equipment in plant. Fire extinguishers (175-2 and 175-3) in the area are at least 4' from any Safety Related equipment, and are not a seismic hazard.
- 8.
Have you looked for and found no other seismic conditions that could Yes adversely affect the safety functions of the equipment in the area?

Have you looked for and found no other seismic conditions that could Yes adversely affect the safety functions of the equipment in the area?
It was noticed there was a fire extinguisher on a short hook, but not within 4' of safety related equipment and is therefore not a seismic hazard.

Plan for Future Seismic Walkdown of Inaccessible Equipment There are 6 items that have been deferred until the next refueling outage (RFO). Table E-I summarizes the reasons each item is inaccessible during normal plant operation, and notes the planned Seismic Walkdown date. There are also 6 Area Walk-Bys associated with these items. Condition Report CR-2012-008440 has been written to schedule the Seismic Walkdowns (and Area Walk-bys) for these items.
All items will be walked down before or during the next RFO, which occurs in the Spring 2014. An updated submittal report will be provided within three months of the completion of this outage.

Cabinets that have been identified as requiring these supplemental internal inspections are those with doors or panels with latches or thumbscrews and can be readily opened during normal maintenance activities. Internal inspections for these cabinets is to be completed for each one in accordance with the station work management scheduling in order to complete them all before the end of the next refueling outage. CR-2012-009635 and Work order C92057302 has been initiated to perform the internal inspections.
The Seismic Walkdown Checklists (SWC) for the 17 equipment items identified in Table E-2 that can be opened for internal inspections will be revised at the time of the supplemental walkdown to indicate the results of these internal inspections. In addition, for items 94, 4 and 7 (2DMS*MCCAI,2EHS*MCCI02, and 2ENS*SWG101), the internal anchorage needs to be verified to the design drawings. The walkdown only verified configuration compliance with the external anchorage. It is noted that the following Class 20 equipment items were opened during the walkdowns:
2CEC*PNL622, 2CEC*PNL859, E-2
ATTACHMENT 5 INACCESSIBLE EQUIPMENT AND PEER REVIEW 2CEC*PNL894, 2CEC*PNL828, 2CEC*PNL896, 2CES*IPNL406 and 2CES*IPNL407. There were no issues identified for these items as a result of the internal inspections.
Internal inspections were completed and therefore, are not included in Table E-2 of equipment items that require additional inspections.

Peer Review Checklist Peer Review Checklist for SWEL - Nine Mile Point Unit 2 Instructions for Completing Checklist This peer review checklist.may be used to document the review of the Seismic Walkdown Equipment List (SWEL) in accordance with Section 6: Peer Review. The space below each question in this checklist should be used to describe any findings identified during the peer review process and how the SWEL may have changed to address those findings. Additional space is provided at the end of this checklist for documenting other comments.
- 1. Were the five safety functions adequately represented in the SWEL 1 selection?
YZ N[-]
The basis for how selection of equipment for the SWEL I is described in the SWEL Scope Development document and the basis includes a description that indicates that the 5 safety functions are represented. However, the information provided on the SWEL does not include the safetfunction(s) supported by each item.
- 2. Does SWEL I include an appropriate representation of items having the following sample selection attributes:
- a. Various types of systems?
- b. Major new and replacement equipment?
- c. Various types of equipment?
- d. Various environments?
It appears that the equipment on the SWEL is located in various environments.
Although not specifically required by the EPRI TR 1025286, it appears that equipment in the Drywell is not represented Y[N NE-I Y[M N[--]
