In addition, we provided the 25 question SRO exam one week early. This is not included in this submittal on paper but is in the folder titled 1 1LT35 Written on the CD-R and has not changed since our early submittal. Also included in the early submittal was two (2) JPMs. These have been included in printed paperwork for your ease of review.

These files represent the written exam as modified from the original submission based upon our discussions, reviews and Operations Management review of those changes.
Also, included is a password protected electronic copy of the ES-401-4.

The Form ES-201-3, Examination Security Agreement, has not been completed but a working copy of the original is included. The final signed original will be sent at a later date when all signatures have been acquired.

- A copy of the ES-201-3, Examination Security Agreement.
- A SCANTRON form Marked as KEY and an LXR TEST KEY report.
- Preliminary grades (RO, SRO-only, SRO-OVERALL scores).
- Ten (10) original exam answer SCANTRON sheets: 6 SRO & 4 RO for use in grading the exam and copies of the same.
- A copy of each applicants LXR generated individual scores report attached to his answer sheet.
- NMP-TR-423-F15 which list the questions asked by the applicants during the exam with the responses given.
- Individual question sheets that the students filled out and the proctor returned to the student with the answer.
- List of Applicants and docket numbers.
- Seating chart utilized during the written exam.
- One entire 100 question exam key with feedback.
- Compilation of Comments collected from the applicants from the post-exam review with facility response/action stated. This document also identifies the facility reviewed HIGH-Miss questions.
