MILLSTONE POWER STATION, UNIT 3 - REQUEST FOR ADDITIONAL INFORMATION REGARDING END OF CYCLE 14 STEAM GENERATOR TUBE INSERVICE INSPECTION REPORT (TAC NO. ME8842)

Dear Mr. Heacock:

By letter dated April 6, 2012 (Agencywide Documents Access and Management System (ADAMS) Accession No. ML12109A044) , Dominion Nuclear Connecticut, Inc. (the licensee),

submitted information summarizing the results of the 2011 steam generator tube inspections at Millstone Power Station, Unit 3. These inspections were performed during the fourteenth refueling outage.

The U.S. Nuclear Regulatory Commission staff has reviewed the information provided by the licensee and has determined that the enclosed request for additional information (RAI) is needed in order to complete the review. A response to this RAI is requested to be provided within 60 days.

UNIT 3 DOCKET NUMBER 50-423 By letter dated April 6, 2012 (Agencywide Documents Access and Management System (ADAMS) Accession No. ML12109A044) , Dominion Nuclear Connecticut, Inc. (the licensee),

submitted information summarizing the results of the end of cycle 14 steam generator tube inspections at Millstone Power Station, Unit 3. These inspections were performed during the fourteenth refueling outage. In order to complete its review of the documents listed above, the staff needs the following additional information:

1. Page 3 of Enclosure 1 to your April 6 letter indicates that secondary side activities were performed in all SGs, and secondary side examinations were performed in SG D.

Please discuss the findings of these activities and examinations.

2. Table 2 indicates that visual examinations of the plugs were performed during 3R12.

Please provide the results of these examinations.

3. Table 7 indicates that a tube with foreign object wear was stabilized and plugged, since a secondary side inspection could not be performed in the area of the degraded tube.

One page 3, you state that the augmented sample inspections with the rotating coil probe included bounding examinations for possible loose part indications and new volumetric flaw (fabrication or foreign object wear) detections. Presumably, these augmented sample inspections included the area around the tube that was stabilized and plugged. Please discuss the following:

a. The scope of the augmented sample inspections performed in the area of the tube that was stabilized and plugged.
b. The technical basis used for your operational assessment, assuming that the loose part is still in the SG.
