NRC Inspection Findings Quad Cities Green - Quad Cities failed to assess and manage maintenance risks resulting in a manual reactor scram on Unit 2.

Green - Quad Cities Unit 2 operators changed operating modes when all required equipment was not operable, specifically an instrument that provides a scram signal on low condenser vacuum.
