Description:

In preparation for work inside the Emergency Service Water vault, water found inside the vault was pumped into drums and sampled for gamma emitters, tritium, and pH. There were no gamma emitters identified, pH was 7.62 and tritium was 102,000 pCi/I. The reporting threshold to the New Jersey DEP for tritium is 2,000 pCi/l.

Immediate actions taken:

The New Jersey DEP was notified of the sample results and a potential.

release of tritium to the soil. It was also iterated that the water was contained in a concrete vault and being pumped into drums.

A prompt investigation (PINV) was initiated.

Additional samples were obtained for tritium analysis - drum of original water from vault, drum of current water being transferred from vault, and a sample from the vault.

Normancleau Associates were contacted to pick up duplicate samples for analysis by Teledyne Brown Engineering.

Recommended Actions:

Complete PINV. Identify potential sources.

During the preparation of the cable replacement for Emergency Service Water (ESW) pump motor 1-2, water was found in the Intake Structure cable manhole (MH-731-1).

Intake Structure manhole provides a structure through which several medium voltage (4160V) and low voltage cables are routed. These cables provide power and control to various equipments located at the Intake Structure area such as 4160V rated ESW pump motors, Circulating water pump motors, USS 1A3, USS 1B3. Circulating Water pump motor and ESW pump motor cables are spliced in this manhole. The manhole is estimated to be about 12 feet deep. Water level was estimated to be about 10 inches deep. All medium voltage cables and cable splices were reported to be above water level and thus not immersed. All previous inspections had indicated a water level of about 2 to 3 inches.

In addition to the ESW 1-2 cable replacement, efforts are underway to replace cables for ESW 1-1. All replacement cables will consist of continuous cable runs, with no cable splices.

Cable duct D-102 was observed to be leaking water in the manhole. This duct is located at the bottom of the East wall of the manhole. It is in the middle of the three ducts at this location. A review of the physical drawings (BR 3180, section N-N and BR 3179) and the cable data information indicates that following circuits are routed through this duct:

1. 21-1844, interlock from service water pump 1-1 to chlorination controls.
2. 21-1845, interlock from service water pump 1-2 to chlorination controls.

Each circuit is 2/C, #12AWG control cable, 120VAC. Insulation is rated for 600V.

Duct D-102 is routed adjacent to the condensate storage tank and the associated piping.

There are no visible signs of any degradation or damage to the cable jackets/insulation that could adversely affect the cables from performing their function.

Water was pumped from the manhole into drums and sampled by Chemistry. The samples tested positive for tritium. The sample findings and subsequent actions are documented in IR 907846. Cables were inspected visually by the work group.

Recommended Actions:

No new action is recommended at this point. All medium voltage cable manholes including the subject manhole are inspected on a periodic basis for the presence of water and pumping out where required. Reference PM73107E. This inspection is part of the Oyster Creek license renewal commitment. The commitment requires inspection at a frequency of 2 years.

However, as of March 2009, the frequency has been increased to a quarterly, basis.until a trend for water accumulation can be established that would determine future recommended actions.

The root cause of the water leak is being tracked under IR 907846.

What are the consequences?

1. Medium voltage cables.

Based on the visual inspection of the cables, the cable insulation/jacket was found to be in good condition. All cable jacket and insulation materials are water resistant. All medium voltage cables including the 4160V cables routed through the subject manhole are tested periodically.

Latest test results indicate that all cables are in a satisfactory condition. Periodic testing of the 4160V cables has been in place at Oyster Creek since early 1990s. The periodic testing will continue as part of the OC License renewal commitment. The current testing program includes testing of the 4160V, 13.8KV (SBO) and 34.5KV (Start up X-former) power feed cables. Most commonly used testing methodology is the on-line partial discharge (cablewise) test.

2. Low voltage cables:

The cables are control or power application with the insulation/jacket rating of 600V. Historically, moisture has not caused failure of such cables due to the following reasons.

The cables are rated for 90 degrees C. Typically they carry very small percentage of their current values. Therefore, they do not experience any significant heating.

The voltage level is significantly lower than the insulation rated value of the cables. Hence the insulation stress level is low.

Typically the cables are non-shielded.

EPRI report TR-103834-P1-2, 1994, page 2.1-2, (Effect of moisture on Life of Power Plant cables) conducted a survey of several users and manufacturers of low voltage power cables. While excavating, in an East-West direction along the Condensate Transfer Pump house North wall, A leak was discovered in the 8-inch diameter carbon steel, schedule 40, CH-2 Line located approximately three feet below grade. The leak is located at the bottom of the pipe and is estimated to be 5/8 inch in diameter.

UT was performed on the pipe in the vicinity of the leak and nominal was thickness was generally found except for an area radiating about three to four inches from the hole where significant corrosion was found.

Immediate actions taken:

OCC notified Pipe was ultrasonically tested to characterize pipe condition.

Temporary clamshell seal applied to pipe which stopped the leak.

Description:

NOS followed up on a false positive tritium analysis on a sample from the main condener discharge that was reported over the weekend and found that Chemists are deviating from expected logkeeping standards. Records of the tritium analysis do not accurately indicate the minimum detectable activity (MDA) levels.

The tritium log was reviewed and deficiencies were noted in the data records. Sample times were not recorded as required by the procedure.

The log sheet had information written into the margin at the bottom of the page that indicated three different MDA levels, depending upon the sample count time. The chemist was recording only < in the results column, where the activity level should be recorded. When asked which MDA applied to which samples, the chemist explained that it depended upon the length of time the sample was counted.. NOS pointed out that the count time is not recorded on the log, and it is impossible to discern which MDA applies to each sample. The technician then recorded < (50 min) in the column where uCi/mL should be recorded. Inaccurate logkeeping could cause inaccurate reports to regulators.

The chemist pointed out that the false positive occurred because the sample was filtered and not distilled and counted for only 10 minutes to obtain a quick indication of the tritium level. Environmental samples require a 50 minute count time to achieve the required MDA level. The chemist indicated that the sample may have had bioluminescence, which has caused false positive results in other samples in the past. When asked how to avoid this interference, he stated that the sample can be distilled to avoid bioluminescence interference. He said that filtering was faster and the main condenser discharge sample was not distilled to save time.

When the positive tritium result was found, then the chemist distilled the sample and re-analyzed and no tritium was present. It was rioted that the samples being analyzed on the day after the false positive were distilled and all indicated less than minimum detectable levels.

Description:

An excavation has been made in an area west of the Turbine Building and North of the Condensate Transfer Pump House. In the trench, a 4" pipe is dripping water. Thia IR is to document that the piping is the abandoned remains of the Makeup to the Demineralized Water Storage Tank. The piping was verified to be the abandoned line by the present of a pipe clamp that was installed as a result of MNCR 91-108-10.

After that time a portion of the Make up pipe was relocated to run inside the Condenser Bay (sometime before 1995.. When this pipe was abandoned it is believed that it was simply cut and left in place. No caps were installed. The remainder to the makeup to the Demineralized water storage tank was rerouted above grade via OC-CCD-328376-001. Refer to drawing GU 3E-424-22-1001 Revision 2. When the rest of the pipe was located above grade some of the abandoned pipe was flushed then capped. There is no effidence however that this pipe was capped.

Immediate actions taken:

Verified that pipe is indeed abandoned in place and initiated this IR.

While returning the Condensate Transfer System back into service, a leak (roughly the size of a quarter) was discovered in the Condensate Transfer Building. This line is a 10-inch diameter, carbon-steel, schedule 40, CH-1 line.

Originally, the lirfe was inspected at two locations (F6) and (F5). The locations had the wrappings removed and coatings inspected over two foot sections. Additionally, the location (F6) had UT performed and the lowest reading was 0.322 (C2021108-03) against a criteria of min wall of 0.034.

This event is a part of the initial reporting of tritium at Oyster Creek.

Therefore, no additional reporting is required.

On 04/29/09 tritium samples were taken for support of the tritium ACMP.

One of these sample had known tritium activity. When the samples were distilled the sample with known tritium activity was distilled first after a blank for cleanliness verification. After the sample with known tritium activity was run it contaminated the sample that was run after it.

Immediate actions taken:

The samples were distilled again with a blank and then the samples were run. The samples were distilled from the least amount of tritium based on previous results to the highest amount of tritium based on previous results and then a blank was run to verify cleanliness of the distillation after all samples were complete.

Recommended Actions:

Samples were re-run and results were consistent with what was expected and previous results. Technician involved was briefed on the logistic of sequencing analysis.

On 4/18/09 Guided Wave inspections were performed on buried condensate transfer lines at Oyster Creek, in support of an ongoing leak investigation. Guided wave was intended to help find the leak location as well as determine the extent of condition for the lines in the area potentially contributing to the tritium found in the sampling wells along with other techniques such as UT and visual. Guided wave inspections were followed up with ultrasonic inspections (UT) to verify the results. The following lines were inspected: 6 Condensate transfer discharge (aluminum), 10 diameter Hotwell Level Control to Hotwell (carbon steel), 8 diameter Hotwell Level Control to Hotwell (carbon steel) were inspected with Guided Wave. Guided Wave results indicated severe corrosion on the 8 line, however follow-up UT performed on 4/28/09 found only minor corrosion. On 4/29/09 SIA was called back in to inspect the remained of piping not shot In addition guided wave did not find a hole in the 10 line. The vendor did not indicate that the data was potentially bad and issued the results that were incorrect; they had not identified a through wall hole. The aluminum line had to have the coating completely removed to verify that there was no leakage. The guided wave provided incorrect results.

Note: A similar IR (914149 was issued in the Cantara CAP system)

Immediate actions taken:

The majority of the 8" and 10" buried piping is being replaced (ref. AR A2222268) and thre remaining portions evaluated on tech.eval. A2222268-13.

An OTDM is also being prepared (ref IR 00907846-11) prior to startup from 1F20.

Recommended Actions:

Engineering to work with Cantera Programs Engineering (components) to understand the capabilities and limitations of guided wave and the reason for recieving incorrect results. Also determine the consequences on the buried pipe program.

There were 5 new wells (W-50 to W-54) that were drilled in' response to the Tritium investigation. Chemistry sampling of the wells are being performed weekly to monitor the Tritium levels in these wells. Original samples taken on 4/24/09 from Well 53 (W-53)were less than LLD or detectable and Well 54 (W-54) indicated 7,950 pCi/L. Samples taken on 4/30/09 indicated Wm53 was approximately 9,000 pCi/L and W-54 was less than LLD or detectable. This raised a question on the validity of the data.

Resampling and reanalysis was performed on 5/1/09. W-53 is indicating 13,100. and W-54 is LLD. The results from 4/30/09 were validated. This implies either the original sample was in error or the groundwater plume is moving.

Immediate actions taken:

Resampling and reanalysis were performed as a confirmatory to validate the results.

Recommended Actions:

Continue to follow the ACMP and trend.

Vendor 'support, New Jersey licensed well driller, and hydrogeologist support will be required for well installation.

NOS observed a PORC 09-07 review of OTDM for repairs needed to support restart and operation of the reactor.

The quorum was verified and PORC members asked detailed questions.

The following deficiencies were noted:

- An old version of the Failure Mode Causal Tree FMCT was in the material sent out for preview by the PROC members. Engineering distributed a revised version at the beginning of the PORC meeting and voting members had to review it again to understand the changes.

- Supporting documentation was not made available for PORC members. A drawing of the piping and material types were not provided, and when requested, an engineer left the meeting to get the drawings. The drawing did not accurately indicate the piping runs or material types.

While interviewing Chemistry staff regarding the groundwater monitoring program associated with the condensate leaks from buried pipes during an inspection visit, an NRC inspector recommended that an additional off-site well in the Kirkwood Aquifer (W-4K) be included in the monitoring program.

The inspector understands that there is no technical reason to believe that the Kirkwood Aquifer has been contaminated. However, from a public relations point of view, he feels that we should include this Kirkwood monitoring well in our sampling program in order to provide assurance to the public that the on-site groundwater contamination is not migrating to off-site wells.

Documentation of External OPEX reviews for ACEs and CCA were less than adequate for 10 of 13 products reviewed during the NOS CAP audit.

Specifics were as follows:

1. Review of ACE 835031, Construction Equipment Comes in contact with Live 480V line, found that external operating experience (OPEX) was not documented as being reviewed as part of the investigation. Internal OPEX that was reviewed did not document the relevance to the event being investigated or how/if the OPEX was used to create corrective actions for this event. The investigator was contacted and stated that he reviewed external OPEX using the INPO website and did not find any relative OPEX (this was not documented in the ACE) and the internal OPEX reviewed was also found to be not applicable (this was not documented in the ACE). As a result there is inadequate documentation to support the review of internal and external OPEX in accordance with the requirements of LS-AA-125-1003, Apparent Cause Evaluation Manual, Attachment 1 for the documentation of previous events.
2. OPEX outside of the Oyster Creek station was not evaluated by ACE 757751, INPO AFI ER.2-1 PM Program Weaknesses. Only two recent Oyster Creek IRs were identified in the previous event review section of the ACE.

No non-Oyster Creek or non-Exelon OE was identified in the ACE.

3. ACE 876836, Oyster Creek Work Package Quality Issues, did not evaluate OPEX outside of Exelon. The ACE listed Oyster Creek INPO AFI and a fleet wide IR on planning issues in the previous events review section of the ACE.
4. ACE 842131, Electrical Flash in the 480 V room, did not evaluate OPEX outside of Exelon. The ACE listed the Oyster Creek and TMI IRs on flash issues in the previous event review section of the ACE.
5. No OPEX was found on ACE 869839, FOF Drill Objectives Not Met.
6. ACE 878594, Reportable Leak from Iso Condenser Vent, did not evaluate OPEX outside of Exelon. The ACE listed Limerick and Braidwood IRs on tritium leaks in the previous event review section of the ACE.
7. ACE 841706, OC 1R22 stretch goal of 123 Rem not met, did not evaluate OPEX outside of Exelon. The ACE listed the Exelon CCA IRs on 6 of 10 sites not meeting stretch goals in the previous event review section of the ACE.

9. CCA 838412, OC 2008 PCE goal not met, did not evaluate OPEX outside of Exelon. The ACE listed an OC CCA and PCEs in the previous event review section of the ACE.
10. CCA 886953, MM clock resets for 2008, did not evaluate OPEX outside of Exelon. The ACE listed an OC Engineering 2007 CCA for clock resets in the previous event section of the ACE.

Two other CCAs and one ACE that were reviewed had internal and external OPEX listed.

These are deficiencies because use of industry OPEX is required by LS-AA-125-1003, Apparent Cause Evaluation Manual and LS-AA-125-1002, Common Cause Analysis Manual. Specifically, section 4.3.7.1.in both revisions 7 and 8 of LS-AA-125-1003 and LS-AA-125-1002, section 4.4.4 state the following:

Review appropriate databases for site and industry events.

Whenever possible, do not limit the OE review to the station CAP database.

Include other databases (like INPO or NRC) in the OE review for similar events.

Discussed with Regulatory Assurance personnel.

Recommended Actions:

Review this issue with the MRC and department QRTs. Ensure those groups understand that industry OPEX needs to be included in ACEs and CCAs. Also, explain how that OPEX is to be used in proposing corrective actions to address issues that are identified by those products.

Why did the condition happen?

1. NOS observation of MRCs review of ACE 892164, In Storage Maintenance (ISM) activities were not performed on two rotating elements, found that the MRC did not note that the ACE did not evaluate OPEX outside of Exelon.

Thus, a contributing cause for this issue could be a gap in MRC reviews and in the departmental reviews before the product goes to the MRC.

2. This deficiency is repeated from the 2007 audit (see IRs 637963 and 636906)

What are the consequences?

Failure to use external OPEX means that the station does not benefit from solutions used at other plants in the industry.

1. The last revision in 2001 was for Exelon name changes only, and the last substantive revision was in 1999. Given that there may have been ODCM changes in that time gap, and since some sampling requirements and expectations have changed since Oyster Creek and other tritium-related events, it would be prudent to revalidate this procedure against the ODCM and company / Oyster Creek environmental procedures.
2. The procedure needs upgrading to current formatting standards.
3. Most of the procedure references have been superceded.

Description:

In routine sampling of water from the excavation site on 04/14/09, a sample was found to have positive activity for tritium and xenon-135 (Xe-135). The State of New Jersey asked for an evaluation why other fission products were not found.

In Oyster Creek's steam-water cycle, steam containing various radionuclides passes from the reactor pressure vessel through the various turbine stages and after condensing becomes condensate. This condensate is pumped to the condensate demineralizer vessels where impurities, including radionuclides are removed. Thus almost all fission products are removed by the condensate demineralizers therefore they are not normally seen in the CSt samples. However, tritium and noble gases are not removed in the condensate demineralizers. Since January 01, 2009, for the weekly andmost recently daily sampling of the CST, Xe-135 has been found in the CST three times out of 23 samples.

Tritium, being an isotope of hydrogen is incorporated in the water molecule, which can exist as steam or liquid water. The condensate demineralizers do not remove water, and therefore do not remove tritium.

The condensate demineralizers do not remove noble gases either. While the steam-jet air ejectors remove most noble gases and other non-condensable gases prior to the condensate demineralizers, the SJAEs are not 100%

efficient at removing all the noble gases. In the steam-water cycle, downstream of the condensate demineralizers, the condensate can be directed to either feedwater or the condensate storage tank (CST).

The noble gas, xenon-135 (Xe-135) has several unique characteristics:

1. It is a fission product having a half-life of 9.10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br />.

Significantly, Xe-135's fission yield from U-235 is rather high - on the order of 6.54%.

2. It is formed via Isomeric Transition (IT) from Xe-135m (another fission product), which has a half-life of 15.3 minutes.
3. It is also formed from Xe-134 via a neutron, gamma reaction. Xe-134 is one of those fission products that is stable and its fission yield is 7.87.% which is even higher than Xe-135's

As stated above, the condensate demineralizers do not remove noble gases such as Xe-135. Some condensate is automatically transferred to the CST via a level control system and hence, Xe-135 can be placed into the CST as well.

The Xe-135 was, correctly identified because of a decay study that was performed in which the radionuclide followed the decay rate according to its half-life. e Therefore the water found in the pit containing tritium and xenon-135 is not an unexpected result. The condensate demineralizers remove the vast majority of fission products and activation products.

Description:

Currently, sampling the intake structure for large volumes of water to support the tritium ACMP requires chemistry technicians to lift a diamond plate grate between the trash rake grates and the traveling screens. This has been reviewed for safety and personnel are required to wear life vests while sampling and fall pirotection is not required since it is less than six feet to the water surface. Although this task can be performed safely, it is preferred to sample the intake from a hatch located in front of the trash rake grate.

Security has replaced the old rusted lock with an MB1 lock so technicians can access the hatch. The hatch does not open due to physical damage from some unknown event. This hatch needs to be repaired so it will open and close freely. Sampling from this hatch is the safest way to sample the intake structure and should be addressed. Sampling by removing the diamond plate in front of the screens is a safety concern, requires security support daily, an FME concern, and physically challenging, which can be eliminated by repairing this hatch.

A walk down of the Site was conducted to identify environmental vulnerabilities. The following concerns were identified:

- 1. Both USSIA3 and USS1B3 electrical transformers have moats around them with drain valves. The drain valves have no valve numbers and are not locked closed. The drain valves on the SBO Xfmr and EDG waste oil tank are both locked closed. Suggest Valves be identified, labeled and locking closed with MIB key.

2. There is a washout around sampling well MW-54 that was identified in IR 925330. The washout exposes soil, which, if there were some kind of leak, could become contaminated instead of just rocks.
3. There is a Bldg. #3 Ingersoll-Rand air compressor installed on the west side of the bldg. that:

a) has no catch containment under the unit.

b) air receiver valving is not identified or labeled.

4. There are 3 large vinyl looking temporary black moats that contain substantial amounts of water. Not sure who has the responsibility to assure these temporary moats are maintained sampled & drained to minimize worst-case scenarios.

a) Huge Exelon Blue Trailer west of new demineralized Water Storage Tank has 5 inches of water in the SW corner of its moat.

b) 2 moats, 1 for each staged transformer just south of the FIN shop has water.

5. The spill kit station just NE of the Main Fuel Oil Moat is not labeled and does not have a kit checklist.
6. An 11 h.p. hydraulic (what looks like a pave roller) machine has an inadequate containment. The handle-operating unit that contains hydraulic lines has no catch containment directly underneath these lines. The unit and its partial containment should be moved off of the stones on the East side of the site just outside the Jingoli trailer.
7. Security Vehicle 4688 was parked on stones just North of the maintenance bldg. with no occupancy.

As part of the ongoing investigation of the tritiated water that leaked from underground pipes, a new monitoring/pumping well (W-55) was installed in the roadway near the Condensate Storage Tank on 05/26. This new well now needs to be tested by our contractor, Conestoga Rovers & Associates (CRA). The testing includes three phases: 1) Well Development - Pumping the well for a period of approximately 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> to remove excess sediment,

2) Step-Drawdown Test - Pumping the well at various flow rates over an 8-hour period to determine the maximum sustainable flow, and 3) Long-Term Aquifer Pumping Test - Pumping the well at a constant rate for a 48-hour period to evaluate the impact of continuous pumping on the aquifer. All of this work will be performed by our contractor, CRA, in accordance with their proposal dated 05/28/09. Senior Management wishes to have Phase 1 implemented by the end of this week or early next week at the latest. This IR is being written to generate an AR to provide the required plant support and get the work on the schedule.

Description:

As part of the ongoing investigation of the tritiated water that leaked from underground pipes, monitoring well MW-1I-1A was sampled on 05/27/09.

The Chemistry Department filtered and analyzed this water sample in accordance with CY-OC-130-530 "Tritium Analysis Liquid Samples'. The results of the analysis showed 83,000 pCi/L tritium. This result was inconsistent with the facts that all prior samples from this well showed no detectable tritium, and that the well is located in an area not believed to be impacted by the pipe leaks. Suspecting a false positive result due to interferences caused by sediment or micro-organisms in the sample, the sample was distilled and re-analyzed in accordance with CY-OC-130-530. The result of the re-analysis was less than the lower limit of detection.

Immediate actions taken:

Discussed the anomalous result with the Chemistry Supervisor and requested the distallation/re-analysis.

Following the positive sample for elevated tritium level in the ESW cable vault and subsequent elevated sample in a monitoring well, NEI was not formally notified at the email address (GWNotice@nei.org) as required by LS-AA-120, Reportable Event RAD 1.34, Industry Groundwater Protection Initiative (GPI) Voluntary Communication. Environmental stated that NEI was notified; however, a record of emailing the specific email address specified in RAD 1.34 has not been identified. ANS was notified by Reg Assurance as required by RAD 1.34 the morning after the leak was identified and RA has been updating the ANS Account Engineer.

Immediate actions taken:

None required.

Recommended Actions:

Submit an NER regarding this and other lessons learned from implementation of RAD 1.34 for the tritium event.

Tritium Sample analysis were performed on water found in electrical vaults and one pipe vault in support of scheduled cable and pipe inspections.

The work orders are R2139427 and C02019960.

Two out of four vaults were found with Tritium activity:

1. Electrical Vault located near the MIA Xfmr (SBO MH-1) - 102,000pCi/l -

previous inspection performed in February 2009 was LLD - This vault is in the vicinity of the other Monitoring wells in review for the Tritium issues managment team.

2. SE RB pipe vault - located inside of the RCA - 14, 200pCi/l - This vault has had history of Tritium activity in the past - in the vicinity of the ISO condenser vent condensation run off.
3. Switchyard vault on the opposite side of the canals was LLD.
4. The Vault at the CTs was LLD. This is located at the backsite Forked River, Combustion Turbine area There were no Gamma activity in the samples.

Regulatory Assurance performed a reportability review and determined both positive results to be not reportable due to previous reports on the GW tritium inspection and Iso-condenser activities Immediate actions taken:

6/11/09 RCL: Discussed with underground piping engineer. There are PMs in place to inspect pipes in the southeast vault on a 2 year frequency. The tritium water found in the southeast vault has been attributed to prior isolation condenser initiation events and leakage from the isolation condenser vents during normal operations. A clean water source of makeup to the isolation condensers has been installed at the site to eliminate.,
