Entergy Nuclear Operations, Inc. is providing the enclosed steam generator tube inspection report in accordance with Technical Specification 5.6.8. The Technical Specification requires that the report be submitted within 180 days after entry into mode 4 following completion of the inspection. Mode 4 was entered on April 26, 2009.

Summary of Commitments This letter identifies no new commitments and no revisions to existing commitments.

Inspection Summary This report provides the Technical Specifications (TS) section 5.6.8, Steam Generator [SG] Tube Inspection Report, required information.

a) The scope of inspections performed on each SG (Table 1),

b) Active degradation mechanisms found (Table 2),

c) Nondestructive examination techniques utilized for each degradation mechanism (Table 3),

d) Location, orientation (if linear), and measured sizes (if available) of service induced indications (Tables 6A, 68 and 6C),

e) Number of tubes plugged during the inspection outage for each active degradation mechanism (Table 4),

f) Total number and percentage of tubes plugged to date (Table 4),

g) The results of condition monitoring, including the results of tube pulls and in-situ testing (page 16), and h) The effective tube plugging percentage for all plugging in each SG (Table 4).

The TS section 5.5.8b.1 steam generator tube integrity structural integrity performance criterion was satisfied. The TS section 5.5.8b.2 steam generator tube integrity accident induced leakage performance criterion was satisfied.

Leakage did not exceed the 0.3 gallon per minute (gpm) limit. Primary-to-secondary leak rate measured using condenser off-gas (Xenon-1 33 or Xenon-1

35) was <0.001 gpm for operating cycle 20 with a slightly decreasing trend over the cycle.

No leakage was predicted for the 2007 to 2009 operating cycle due to observed SG tube degradation at the 2007 refueling outage (1 R19). All plugs installed in the Palisades SGs post-operation are Westinghouse ribbed mechanical plugs, which are leak-tight for all plant conditions. The Palisades SGs also contain 308 tubes plugged pre-service in SG E-50A, and 309 tubes plugged pre-service in SG E-50B. Of the tubes plugged pre-service, one tube in SG E-50A and four tubes in SG E-50B are plugged using welded plugs, which are leak tight. The remaining tubes plugged pre-service are plugged using Combustion Engineering (C-E) roll plugs, which are also designed to be leak tight for all plant conditions.

Eighteen tubes were plugged in SG E-50A and ten tubes in SG E-50B during the 2009 refueling outage (1 R20). None of the 28 tubes plugged required stabilization. Refer to Tables 5 and 6 for details. No tubes were pulled in either SG. In-situ pressure testing was completed on one tube in SG E-50B Row 72 Column 107 and details are provided after Table 58.

All tubes passed condition monitoring and met both the accident leakage and burst steam generator performance criteria. Meeting the performance criteria 3

provides reasonable assurance that the steam generator tubing remains capable of fulfilling its specific safety function of maintaining Primary Coolant Pressure Boundary.

Background

The Palisades plant has two (2) replacement Combustion Engineering (CE)

Model 2530 steam generators (SGs) that were installed in 1990 refueling outage 1 R08. Each SG has 8219 total tubes fabricated from mill annealed (MA) Alloy 600. The tubes have an outside diameter of 0.75 inches and a nominal wall thickness of 0.042 inches. This most recent SG inspection was the third and final inspection of the third sequential 60 EFPM period since the first lSI performed in 1 R09 in 1992. Table A below provides a summary of the SG inspections performed since replacement.

Active Degradations Mechanisms Table 2 below summarizes the active degradation mechanisms found in the Palisades steam generators.

Circumferential PWSCC in the expanded tube in tubesheet was identified as a potential mechanism by steam generator degradation assessment for 1 R20. No scope expansion was required. These tubes have been removed from service and repaired by tube plugging. Axial PWSCC in expanded tube in tubesheet region was previously reported. Indication depth is shallow and indication arc lengths are short <<45 degrees).

Operability issues as a result of condition monitoring in 1 R20 with the identification of a new steam generator circumferential primary water stress corrosion cracking (PWSCC) damage mechanism have been eliminated by tube plugging.

Steam Generator eddy current testing was completed on April 8, 2009 on Steam Generator E-50A and E-50B. Steam generator eddy current examinations performed during 1 R20 indicated the need to plug eighteen (18) tubes in E-50A and ten (10) tubes in SG E-50B.

Secondary Inspection in the SGs included Annulus and Tube Lane Foreign Object Search and Retrieval (FOSAR)

The possible loose parts in both SGs were reviewed by FOSAR. Condition report CR-PLP-2009-01448 CA-01 Identifies Loose Part in Steam Generator E-50A Eddy Current Inspection. The loose part was identified on the top of Steam 6

Generator E-50A (SG E-50A) hot leg tubesheet. It was in contact with 4 tubes, 1 of which possibly exhibited loose part wear. The tube with wear is in Row 17 Column 4. The loose part was estimated to be approximately 2 inches in length.

Foreign Object Search and Retrieval (FOSAR) attempts were successful in retrieval of the loose part. The loose part was identified as demister banding wire approximately 4 inches in length. SG E-50A tube Row 17 Column 4 was retested and has no mechanical wear identified. SG E-50A tubes Ri7C4, R18C3, Ri8C5 and Ri9C4 were all left inservice after the loose part removed with no further issues.

No loose parts were indentified in SG E-50B. Only small sludge rocks, that broke when retrieval was attempted, were identified.

Nondestructive Examination Techniques Beginning on the next page, Table 3 lists the nondestructive examination techniques utilized for each degradation mechanism.

Tables 6A, B & C at the end of this report provide listings of service induced indications with their location, orientation (if linear), and measured sizes (if available). Table 6A lists the service induced indications that required tubes to be plugged plus 3 volumetric indications not at support structures that remain in service. Two are historical loose part wear scars with no foreign object present and the third is attributed to a lap traced to the baseline inspection. Table 6B lists all the indications due to tube wear for tubes that remain in service for SG E-50A and Table 6C lists similar indications for SG E-50B.

Tube Plugging Summary Table 4 provides a summary of the tubes plugged during 1 R20 for each active degradation mechanism in each steam generator. This table also includes the total number and percentage of tubes plugged to date along with the effective percentage for all plugging in each SG. No repairs other than plugging (e.g.

sleeving) have been performed in the Palisades SGs so the effective plugging is equal to the total plugging. Table 5 provides a list of the individual tubes plugged during 1 R20 with Tables 5A and 5B providing SG support and acronym information.

The tube was screened based on amplitude sizing which is conservative as compared to phase based sizing. The test was conducted in accordance with the EPRI Guidelines. A localized tool was used to perform the test. The test was performed in a leak mode for all test pressures, that is, the proof bladder was not used for the proof test pressure (2: 3800 psi). The results of the test were zero tube leakage and no tube burst. Testing pressures which included offsets for temperature and instrumentation uncertainty were:

Normal Delta Pressure (NODP) 1500 Intermediate Pressure 2250 Steam Line Break Pressure 2800 Intermediate Pressure 3800 3NODP 4450 500 psig above 3NODP 5000 1000 psig above 3NODP 5500 The elevated pressure above 3NODP was performed in an effort to quantify the sizing uncertainties associated with calculating burst pressures.

Post RPC data was performed following the in-situ test. The results of that testing indicated no significant change in the flaw characteristics. The volts peak to peak increased from 0.49 to 0.79 volts. Listed below is the terrain map of the post in-situ test with the plus point:

SG E-50B tube Row 72 column 107 was removed from service and repaired by tube plugging due to the original axial outside diameter stress corrosion (ODSCC) flaw identified.

No other SG tubes required in-situ pressure testing and no tubes were pulled.
