RO 2/1 013K1.18 Premature reset of ESF actuation - No discriminating actions on early reset of ESF actuations (Containment Spray, SI, Containment Isolation) since components do not change state. Selected 013K6.01 as replacement.

RO 2/1 013K1.06 K/A is valid for KPS but when selection of Tier 1 was completed K6 had only been selected once while K1 had been selected 7 times (5 times from 2/1). Decided to deselect a K1 and reselect from K6 to meet sampling requirements. Selected 013K6.01 as replacement.

RO 2/1 076A1.02 Monitor reactor and turbine building closed cooling water temperatures - KPS does not have closed cooling loops cooled by SW or similar cooling systems. No other allowable K/As in A1 for 076 so reselected 076 G 2.4.50 as replacement.

SRO 1/2 036AA2.01 ARM indications as related to Fuel Handling incidents - Have seven other questions related to radiation monitors on RO exam and one on SRO exam. Also K/A does not allow for SRO-level question development that is discriminating. Selected 036AA2.03 as replacement, which also is related to radiation monitor/release values but has SRO-level quality attributes.

SRO 2/2 034A4.01 & Monitor radiation levels/Monitor neutron levels from control room.

034A4.02 Have seven other questions related to radiation monitors on RO exam and one on SRO exam. Also K/A does not allow for SRO-level question development that is discriminating. Selected 034K5.03 as replacement.

RO 3 2.1.17 Make accurate, clear and concise verbal reports. Not applicable to written; evaluated in Operating Exam. Selected 2.1.7 as replacement.

SRO 3 2.1.6 Manage control room crew during plant transients - SRO-level but Not applicable to written. Evaluated in Operating Exam. Selected 2.1.3 as replacement.

SRO 3 2.2.40 Apply Technical Specifications for a system - SRO-level but not appropriate for Tier 3 since no system exists. Evaluated in Operating Exam. Selected 2.2.37 as replacement.

SRO 3 2.3.15 Knowledge of radiation monitoring systems, such as fixed radiation monitors and alarms, portable survey instruments, personnel monitoring equipment, etc - Does not allow for SRO-level question development that is discriminating. Selected 2.3.13 as replacement.

RO 1/1 056AK1.04 Definition of saturation conditions, implications - Similar to K/A 010K5.01 in Tier 2/1 on RO outline. Could not develop a dissimilar discriminatory question on this topic. No other K/As in K1 group that would differentiate from the (KAs) questions selected elsewhere on thermodynamics and natural circulation. Reselected from K3 group and selected 056AK3.02 as replacement.

RO 2/1 076AA2.03 Determine or interpret RCS R/A level meter for high RCS activity -

Low discrimination value in that only one meter used (R-9) and indications are either PPCS (Computer) alarm or control board annunciator for alert or high level on R-9. Selected 076AA2.02 as replacement.

RO 2/1 039K1.07 Relationship/cause-effect relation between MRSS and RMS - Large number of radiation monitor questions on written (seven on RO and one on SRO). Also this area is on Operating Exam dynamics with a SG tube leak or SGTR. Selected 039K1.07 as replacement RO 2/1 073A4.03 Operate Check Source for Rad monitor - This K/A is selected as a JPM on the Operating exam. Selected 073A4.01 as replacement.

RO 2/2 034A1.02 Predict or monitor limits on changes in water level in refueling canal

- Not discriminatory for RO-level only. Related to KA already selected in Tier 1/2 (036AK2.02). No other 034A KA exist that can be used. Selected 034 2.1.32 (G) as replacement.

SRO 1/2 036A2.03 Magnitude of potential R/A release from fuel handling incident -

Large number of questions related to radiation monitors (six on RO and one on SRO), and large number of fuel handling related KAs (two on RO and three on SRO). Selected new E/APE W/E01EA2.2 as replacement

Tier / Randomly Reason for Rejection Group Selected K/A RO 2/2 028A1.01 According to USAR 5.8.2: Although generation of hydrogen can follow a LOCA, the postulated hydrogen release from a design basis LOCA has been determined not to be risk significant because it is not large enough to lead to early containment failure. The risk associated with hydrogen combustion is beyond design basis (e.g.,

severe) accidents. 10CFR50.44 as revised September 16, 2003, eliminated the hydrogen release associated with a design basis LOCA. Question would ask for information in the SAMG. Selected new K/A 028K5.03 SRO 1/1 057G2.4.1 Not credible. Entry into EAL for a Loss of Vital AC Inst. Bus would require a loss of 3 of the 4 Vital AC Inst. Buses. With the plant redundancy not a credible situation. Selected new K/A 057G2.4.11

Instructions

[Refer to Section D of ES-401 and Appendix B for additional information regarding each of the following concepts.]

1. Enter the level of knowledge (LOK) of each question as either (F)undamental or (H)igher cognitive level.
2. Enter the level of difficulty (LOD) of each question using a 1 - 5 (easy - difficult) rating scale (questions in the 2 - 4 range are acceptable).
3. Check the appropriate box if a psychometric flaw is identified:
  • The stem lacks sufficient focus to elicit the correct answer (e.g., unclear intent, more information is needed, or too much needless information).
  • The stem or distractors contain cues (i.e., clues, specific determiners, phrasing, length, etc).
  • The answer choices are a collection of unrelated true/false statements.
  • The distractors are not credible; single implausible distractors should be repaired, more than one is unacceptable.
  • One or more distractors is (are) partially correct (e.g., if the applicant can make unstated assumptions that are not contradicted by stem).
4. Check the appropriate box if a job content error is identified:
  • The question is not linked to the job requirements (i.e., the question has a valid K/A but, as written, is not operational in content).
  • The question requires the recall of knowledge that is too specific for the closed reference test mode (i.e., it is not required to be known from memory).
  • The question contains data with an unrealistic level of accuracy or inconsistent units (e.g., panel meter in percent with question in gallons).
  • The question requires reverse logic or application compared to the job requirements.
5. Check questions that are sampled for conformance with the approved K/A and those that are designated SRO-only (K/A and license level mismatches are unacceptable).
6. Enter question source: (B)ank, (M)odified, or (N)ew. Check that (M)odified questions meet criteria of ES-401 Section D.2.f.
7. Based on the reviewers judgment, is the question as written (U)nsatisfactory (requiring repair or replacement), in need of (E)ditorial enhancement, or (S)atisfactory?
8. At a minimum, explain any U ratings (e.g., how the Appendix B psychometric attributes are not being met).
