DOCKET NO. 50/395 OPERATING LICENSE NO. NPF-12 SPECIAL REPORT (SPR) 09-0001 South Carolina Electric & Gas Company (SCE&G) is submitting this report pursuant to the requirements of Technical Specification (TS) 3.3.3.6 a., Table 3.3-10, Items 6 and 11. This report addresses the inoperability of the Reactor Coolant Loop "C" Hot Leg Temperature Element ITE00433. The temperature element supports part of the accident monitoring instrumentation required by NUREG-0737 item II.F.2.

ITE00433 was declared inoperable June 12, 2009, at 0430 hours0.00498 days <br />0.119 hours <br />7.109788e-4 weeks <br />1.63615e-4 months <br /> due to spurious alarms.

VCSNS diagnostics indicated the cause of the erroneous readings is associated with the element and not with supporting components. Preplaned alternate methods of obtaining accurate readings are provided through the remaining channel of Reactor Coolant Outlet Temperature (Wide Range) and Reactor Vessel Level Instrumentation System. ITE00433 is a dual element RTD. Its sister element ITE00433A is operable and provides Reactor Coolant System Hot Leg "C" wide range temperature indication at the Control Room Evacuation Panel (CREP) and Technical Support Center (TSC). Due to the location of the element, restoration of channel operability cannot be completed until the next available outage.
