Enclosed is one copy of the Final Report for the October 7, 2008, Radiological Emergency Preparedness (REP) Full Participation Plume Exposure Pathway Exercise for the Perry Nuclear Power Plant. The State of Ohio, Lake, Geauga and Ashtabula Counties, and the utility owner/operator, FirstEnergy Nuclear Corporation, participated in this exercise.
No Deficiencies were identified for any jurisdiction during this exercise. There were no Areas Requiring Corrective Action (ARCAs) identified for the State of Ohio and Ashtabula County during this exercise.
There were no previous ARCAs for the State of Ohio, Lake, Ashtabula and Geauga Counties that were resolved.
There was one ARCA each identified for Lake and Geauga Counties. The ARCA identified for Lake County was issued under Criterion 3.a. 1 - Implementation of Emergency Worker Exposure Control, whereby a Wickliffe Firefighter was not familiar with the permanent record dosimeter (PRD) that was issued in his dosimetry kit during an out-of-sequence demonstration. This issue was successfully re-demonstrated.
The ARCA identified for Geauga County was issued under Criterion 6.b. 1 - Monitoring and decontamination of emergency worker equipment including vehicles, whereby a Chardon Firefighter operating the survey meter moved it too rapidly and held it at a distance that did not meet the appropriate requirements. This issue was successfully re-demonstrated.
A detailed discussion of these issues can be found in Part IV of the Final Report.
Based on the results of the October 7, 2008, exercise, the offsite radiological emergency response plans and preparedness for the State of Ohio and affected local jurisdictions, site-specific to the Perry Nuclear Power Plant, can be implemented and are adequate to provide reasonable assurance that appropriate measures can be taken offsite to protect the health and safety of the public in the event of a radiological emergency at the site.

Therefore, the Title 44 CFRPart 350, approval of the offsite radiological emergency response plans and preparedness for the State of Ohio site-specific to the Perry Nuclear Power Plant, granted on June 17, 1994, remain in effect.
Copies of this report have been provided to the DHS/FEMA National Office, Nuclear Regulatory Commission (NRC) Region III, and the State of Ohio.

- 1. Executive Summary On October 7, 2008, a Radiological Emergency Preparedness (REP) Full Participation Plume Exposure Pathway Exercise was conducted for the 10-mile Emergency Planning Zone (EPZ) around the Perry Nuclear Power Plant by the U.S. Department of Homeland Security (DHS), Federal Emergency Management Agency (FEMA). The purpose of the exercise was to assess the level of preparedness of State and local jurisdictions in responding to a radiological emergency. This exercise was held in accordance with DHS/FEMA policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures.
The most recent exercise at this site was conducted on October 24, 2006. The qualifying emergency preparedness exercise was conducted on November 28, 1984.
DHS/FEMA wishes to acknowledge the efforts of the many individuals in the State of Ohio, and Lake, Ashtabula and-Geauga Counties, who participated in this exercise along with the State government.
Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities. Cooperation and teamwork on the part of all participants was evident during this exercise.
This Draft Report contains the evaluation of the biennial exercise and the evaluation of the following out of sequence activities:

- 2. Introduction On December 7, 1979, the President directed FEMA to assume the lead responsibility for all offsite nuclear planning and response. DHS/FEMA activities are conducted pursuant to Title 44 Code of Federal Regulations (CFR) Parts 350, 351, and 352.
These regulations are a key element in the Radiological Emergency Preparedness (REP) Program that was established following the Three Mile Island Nuclear Station accident in March 1979.
The FEMA Title 44 CFR 350 establishes the policies and procedures for DHS/FEMA initial and continued approval of State and local governments' radiological emergency planning and preparedness for commercial nuclear power plants. This approval is contingent, in part, on State and local governments' participation in joint exercises with licensees.
DHS/FEMA responsibilities in radiological emergency planning for fixed nuclear facilities include the following:
- Taking the lead in offsite emergency planning and in the review and evaluation of RERPs and procedures developed by State and local governments;
- Determining whether such plans and procedures can be implemented on the basis of observation and evaluation of exercises of the plans and procedures conducted by State and local governments;
- Responding to requests by the U.S. Nuclear Regulatory Commission (NRC) pursuant to the Memorandum of Understanding between the NRC and FEMA dated June 17, 1993 (Federal Register, Vol. 58, No. 176, September14, 1993); and
- Coordinating the activities of Federal agencies with responsibilities in the radiological emergency planning process:
- U.S. Nuclear Regulatory Commission,
- U.S. Environmental Protection Agency,
- U.S. Department of Energy,
- U.S. Department of Health and Human Services,
- U.S. Department of Transportation, 4
- U.S. Department of Agriculture,
- U.S. Department of the Interior, and
- U.S. Food and Drug Administration.
Representatives of these agencies serve on the DHS Regional Assistance Committee (RAC), which is chaired by DHS/FEMA.
Formal submission of the RERPs for the Perry Nuclear Power Plant to DHS/FEMA Region V by the State of Ohio and involved local jurisdictions occurred on January. 23, 1986. Formal approval of these RERPs was granted by FEMA on June 15, 1987, under 44 CFR 350.
A REP Full Participation Plume Exposure Pathway Exercise was conducted on October 7, 2008, by DHS/FEMA to assess the capabilities of-State and local emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving the Perry Nuclear Power Plant. The purpose of this exercise report is to present the exercise results and findings on the performance of the offsite response organizations (ORO) during a simulated radiological emergency.
The findings presented in this report are based on the evaluations of the Federal evaluator team, with final determinations made by the DHS/FEMA RAC Chairman, and approved by DHS/FEMA Headquarters.
The criteria utilized in the DHS/FEMA evaluation process are contained in:
- NUREG-0654/FEMA-REP-1, Rev. 1, "Criteria for Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980;
- FEMA-REP-14, "Radiological Emergency Preparedness Exercise Manual," September 1991; and
- FEMA "Radiological Emergency Preparedness: Exercise Evaluation Methodology," as published in the Federal Register Notice/Vol. 67, No. 80, dated April 25, 2002.
Section III of this report, entitled "Exercise Overview," presents basic information and data relevant to the exercise. This section of the reportcontains a description of the 5
plume pathway EPZ, a listing of all participating jurisdictions and functional entities that were evaluated, and a tabular presentation of the time of actual occurrence of key exercise events and activities.
Section IV of this report, entitled "Exercise Evaluation and Results," presents detailed information on the demonstration of applicable exercise criteria at each jurisdiction or functional entity evaluated in a jurisdiction-based, issues-only format. This section also contains: (1) descriptions of all Deficiencies and ARCAs assessed during this exercise, recommended corrective actions, and (2) descriptions of resolved ARCAs assessed during previous exercises and the status of the OROs' efforts to resolve them.

Exercise Overview Contained in this section are data and basic information relevant to the October 7, 2008, REP Full Participation Plume Exposure Pathway Exercise to test the offsite response organizations emergency capabilities in the area surrounding the Perry Nuclear Power Plant. This section of the exercise report includes a description of the plume Emergency Planning Zone, and a listing of all participating jurisdictions and functional entities that were evaluated, and a tabular presentation of the time of the actual occurrence of key exercise events and activities.
3.1. EPZ Description The Perry Nuclear Power Plant (PNPP) reactor units one and two are located on the southeast shoreline of Lake Erie in Lake County, Ohio. The site is approximately seven miles northeast of Painesville, Ohio and 35 miles east of Cleveland, Ohio.
The plant site occupies approximately 1100 acres on a lake plain 50 feet above the lake low-water datum. The terrain surrounding the plant is essentially flat within five miles of the lakeshore. Rising generally to the south, the site is broken only by a lattice work of shallow, north-south stream beds and low, almost unnoticeable east-west ridges; the latter are remnants of ancient lake-shores. About five miles inland, the shoreline plane is abruptly cut by a 100-foot-deep ravine of the Grand River. The terrain south of the river is more rolling, interrupted by north-south ravines of tributaries of the Grand, Kellogg, Big, Paine, and Mill creeks and several smaller streams. A large portion of the site is forested, some 250 acres are devoted to the plant structural complex and the remainder is open grassland.
Lake County, where the nuclear power plant is located, is the smallest of Ohio's 88 counties, with 232 square miles. The 2000 census approximates the population to be 102,920 residents currently within the 10-mile emergency planning zone (EPZ); 13, 591 are residents of Geneva and Harpersfield Townships in Ashtabula County with an estimated 1,930 in Geauga County.

4.2. Status of Jurisdictions Evaluated This subsection provides information on the evaluation of each participating jurisdiction and functional entity in a jurisdiction-based, issues-only format. Presented below are the definitions of the terms used in this subsection relative to criteria demonstration status.

The following are definitions of the two types of exercise issues that maybe discussed in this report.
- A Deficiency is defined in FEMA-REP-14 as "...an observed or identified inadequacy of organizational performance in an exercise that could cause a finding that offsite 24
emergency preparedness is not adequate to provide reasonable assurance that appropriate protective measures can be taken in the event of a radiological emergency to protect the health and safety of the public living in the vicinity of a nuclear power plant."
- An ARCA is defined in FEMA-REP-14 as "...an observed or identified in-adequacy of organizational performance in an exercise that is not considered, by itself, to adversely impact public health and safety."
DHS/FEMA has developed a standardized system for numbering exercise issues (Deficiencies and ARCAs). This system is used to achieve consistency in numbering exercise issues among DHS/FEMA Regional Offices and site-specific exercise reports within each Regional Office. It also expedites tracking of exercise issues on a nationwide basis.
The identifying number of Deficiencies and ARCAs includes the following elements, with each element separated by a hyphen (-).
- Plant Site Identifier - A two-digit number, corresponding to the Utility Billable Plant Site Codes.
" Exercise Year - The last two digits of the year the exercise was conducted.
- Criterion Number - An alpha and two-digit number corresponding to the criteria numbers in the six Exercise Evaluation Areas described in Federal Register Notice/Vol.
67, No. 80, April 25, 2002, which amends FEMA-REP 14, Radiological Emergency Preparedness Exercise Manual.
- Issue Classification Identifier - (D = Deficiency, A = ARCA). Only Deficiencies and ARCAs are included in exercise reports. Plan issues are reported to the State(s) via letter from the RAC-Chairman. Therefore, standardized issue numbers are not assigned to Plan Issues.
- Exercise Issue Identification Number - A separate two (or three) digit indexing number, assigned to each issue identified in the exercise.

CONDITION: A Wickliffe firefighter was confused when questioned on the procedures for using his permanent record dosimeter (PRD).
POSSIBLE CAUSE: The cause of this inadequacy appeared to be a training issue. A Controller was present and requested a brief retraining session.
REFERENCE:
NUREG-0654: K.3.a, 3.b REP-14/15: 5.1, 5.2 EFFECT: This would have resulted as a health and safety concern in that the firefighter would not be knowledgeable of recording the possible extended exposure to radiation and the purpose of the permanent read dosimeter would not have been achieved.
CORRECTIVE ACTION DEMONSTRATED: The evaluator conferred with the controller and agreed that the firefighter could be retrained and redemonstrate his knowledge of dosimetry. The lead firefighter then repeated his dosimetry briefing and the controller aided in retraining the firefighter. The firefighter was again interviewed by the evaluator and demonstrated that he did understand the use of the TLD, that it provided a complete record of radiation exposure, that it must be worn outside of his clothing in or near the middle of his torso, and that it would be turned in to the leader at the end of his assignment.

CONDITION: When vehicle monitoring was demonstrated, the probe was initially moved too rapidly and it was held.too far from the surface of the vehicle.

EFFECT: At the speed and distance the probe was used, contamination would not have been detected on the vehicle. Persons could come in contact with the contaminated area and become contaminated themselves without their knowledge.
CORRECTIVE ACTION DEMONSTRATED: The incorrect monitoring process was stopped. The controller completed on-the-spot retraining. The emergency worker successfully redemonstrated monitoring techniques. He held the probe the proper distance from the vehicle and moved it at the proper speed.

APPENDIX 3 EXERCISE OBJECTIVES AND EXTENT-OF-PLAY AGREEMENT This appendix lists the exercise criteria that were scheduled for demonstration in the Perry Nuclear Power Plant REP Full Participation Plume Exposure Pathway Exercise on October 07, 2008, and the off-site extent of play agreement approved by DHS/FEMA on September 29, 2008.
The exercise criteria' contained in FEMA, Radiological Emergency Preparedness Exercise Evaluation Methodology, as published in the Federal Register Notice/Vol 67, No 80, dated April 25, 2002, represent a functional translation of the planning standards and evaluation criteria of NUREG-0654/FEMA-REP-1, Rev. 1, Criteria for the Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants, November 1980.
Because the exercise criteria are intended for use at all nuclear power plant sites, and because of variations among off-site plans and procedures, an extent-of-play agreement is prepared by the State and approved by DHS/FEMA to provide evaluators with guidance on expected actual demonstration of the criteria.

Criteria that can be re-demonstrated immediatelyfor credit, at the decision of the evaluator, include the following: 3.a.1, 3.d.1, 3.d.2, 6.a.1, 6.b.1, 6.c.1 and 6.d.1. Criteria that may be re-demonstrated, as approved on a case-by-case basis by the Chairperson of the Regional Assistance Committee, include the following: 2.a. 1, 2.b. 1, 2.b.2, 5.a. 1 and 5.b.1.
Evaluation Area 1 - Emergency Operations Mananement Criterion 1.a.1: OROs use effective procedures to alert, notify and mobilize emergency personnel and activate facilities in a timely manner.
Extent of Play The State Emergency Operations Center (SEOC) and the Ohio Highway Patrol Dispatch Center will receive initial notification over the 24-hour phone line. Once the SEOC Assessment Room is activated all dedicated phone communications will be received there. SEOC personnel will notify and mobilize State agencies to respond at the appropriate Emergency Classification Level (ECL) in accordance with procedures. The following State Agencies will participate. in the exercise:

Extent of Play The baseline exercise for demonstration of this criterion was conducted in April 2002.

Criterion 1.c.1: Key personnel with leadership, roles for the ORO provide direction and control to that part of the overall response effort for which they are responsible.
Extent of Play Direction and control of state activities will be demonstrated at the SEOC. The Executive Director of Ohio EMA will be positioned in the SEOC Executive Room and will coordinate decisions on behalf of the Governor's office. The Operations Branch Director/Operations Section Coordinator will conduct periodic briefings for the SEOC Operations Room staff. The Ohio Department of Health is responsible for determining the State PAR in the SEOC Assessment Room and will provide periodic briefings to the Executive Room.
Criterion 1.d.1: At least two communication systems are available and operate properly and communication links are established and maintained with appropriate locations.
Communication capabilities are managed in support of emergency operations.
Extent of Play The primary means of communications between the State EOC and the County EOCs, forward JIC (Lakeland' Community College), and the EOF is by commercial and/or dedicated telephone. Backup communications (radio and/or cellular phones) will be demonstrated.
Criterion 1.e.1: Equipment, maps, displays, dosimetry, potassium iodide (KI) and other supplies are sufficient to sulpport emergency operations.
Extent of Play The state will demonstrate the use of equipment, maps and displays to support emergency operations. The maps traditionally used in the EOC will be available and used for exercise evaluation purposes.
Evaluation Area 2 - Protective Action Decision Making Criterion 2.a.l: OROs use a decision-making process, considering relevant factors and appropriate coordination, to insure that an exposure control system, including the use of KI, is in place for emergency workers including provisions to authorize radiation exposure in excess of administrative limits or protective action guides.
Extent of Play The SEOC Assessment Room staff will consider PAGs and Administrative Limits to develop recommendations, including KI, based on their technical evaluation of the available data and provide to the SEOC Executive Room. Recommendations will be forwarded to the SEOC Executive group and subsequently to the County EOC's (Lake, 59
Ashtabula, and Geauga). County EOCs Will disseminate recommendations to their emergency workers. Field monitoring data will be provided to the SEOC Assessment Room via controller injects.
The State Field Monitoring Teams (FMT's) will be demonstrated out of sequence on October 6.

The "send" button on the Lake County EAS encoder will be depressed and this action will activatethe outgoing alert light and playback of the recorded message. Also, a data sheet will automatically be printed by the encoder and the time recorded on the data sheet. Ashtabula and Geauga counties have EAS equipment (encoder) and could perform this fuinction, if necessary..
Lake County will fax a copy of the EAS message(s) to the Joint Information Center (JIC) where Public Information Officers (PIOs) may distribute hard copies to the news media representatives and may, if time and circumstances permit, make an announcement regarding the message. Lake County will FAX the SNB(s) to the Joint Information Center (JIC) where one of the counties' PIOs will represent the three counties by directly proceeding to the briefing room and reading the contents of the message to the assembly of news media representatives and answering any questions pertaining to the message. Upon a protective action decision (PAD) by the combined Executive Groups of Ashtabula, Lake and Geauga counties, the Lake County EOC Staff will prepare the appropriate Emergency Alert System (EAS) message and simulate sounding the sirens and sending the message over the EAS system. A quiet siren test will be demonstrated by the Lake County EOC in lieu of an actual siren sounding.
Simulation of sending the EAS message(s) will be conducted as follows:
The procedure for the EAS encoder will be followed to deliver the message to WTAM, the Local Primary Station, with the exception that the telephone number to the station will not be dialed. By not connecting to WTAM, there is no possibility of accidentally broadcasting the message over one or more of the participating EAS stations.
The "send" button on the Lake County EAS encoder will be depressed, and this action will activate the outgoing alert light and playback of the recorded message. Also, a data sheet will automatically be printed by the encoder and the time recorded on the data sheet. Ashtabula and Geauga counties have EAS equipment (encoder) and could perform this function, if necessary.
Lake County will fax a copy of the EAS message(s) to the Joint Information Center (JIC) where Public Information Officers (PIOs) may distribute hard copies to the news media representatives and may, if time and circumstances permit, make an announcement regarding the message. Otherwise, the EAS message can be announced at the next scheduled press briefing or in response to news media inquiries about the PAD message.
In delivering information about the EAS message just released, the counties' PIOs may indicate that a corresponding Special News Bulletin (SNB) will be issued soon. Lake County will also fax a copy of the EAS message to Ashtabula and Geauga counties' EOCs.
Criterion 5.a.2: (RESERVED)
Criterion 5.a.3: Activities associated with FEMA approved exception areas (where applicable) are completed within 45 minutes following the initial decision by authorized offsite emergency officials to notify the public of an emergency situation. Backup alert and notification of the public is completed within 45 minutes following the detection by the ORO of a failure of the primary alert and notification system.

SNB's give the same message as their corresponding EAS message except SNB's provide greater detail. Lake County will FAX the SNB(s) to the Joint Information Center (JIC) where one of the counties' PIOs will represent the three counties by directly proceeding to the briefing room and reading the contents of the message to the assembly of news media representatives and answering any questions pertaining to the message. 2008 Evaluated Exercise SCENARIO TIMLINE 10/07/08-0930 Event 5:
Weakened from previous seismic activity, a Main Steam line develops a leak downstream of the primary containment isolation valves in secondary containment. Main Steam Line A is unable to be isolated due to coincidental failures caused by the earthquake.
A SITE AREA EMERGENCY is declared due to:
A challenge to the Reactor Coolant System Barrier and a loss of the Containment Barrier result in Site Area Emergency classification EAL AS3.
Additionally, radiological conditions begin to deteriorate, but do not meet Clad Damage criteria of EPI-B 13.
Eventually, an event related, unfiltered, nuclear steam release into the environment through the Turbine Building Heater Bay vent path develops. Resultant dose projections result at the Site Boundary exceeding the entry criteria for Site Area Emergency classification EAL HSI.

The Technical Support Center (TSC) Security Coordinator calls the Lake County EOC and requests traffic control to assist with evacuation of non-essential personnel from the PNPP Site. Evacuation of non-essential personnel is simulated.
ODH and ODA make an initial recommendation for livestock and poultry.
Consolidated Railways (CONRAIL) and Norfolk &
Southern Railways are notified to restrict rail traffic in the 10-Mile EPZ.

2008 Evaluated Exercise SCENARIO TIMLINE 10/07/08-1100 Event 6:
A GENERAL EMERGENCY (GE) is declared due to:
Seismic damage and abnormal rod configuration result in further degradation of the fuel cladding.
Drywell radiation levels of greater, than 4 R/hr indicate cladding damage per EPI-B13. This is a direct loss of the Fuel Clad Barrier and results in a General Emergency classification AG1.
The indication of clad damage per EPI-B13 also toggles the isotopic makeup in the Computer Aided Dose Assessment Program (CADAP).
Resultant dose projections result at the Site Boundary exceeding the entry criteria for General Emergency classification EAL HG1.
Although either EAL classification is valid, it is expected that the barrier failure will lead the dose projection classification.
State Assessment -team receives notification of the General Emergency and takes the utility's recommendations under advisement. The team formulates the State's protective action recommendation, obtains the Governor's (or designated representative in EOC) approval and informs the counties of the Governor's recommendations via the EDL Counties' Executive Groups are notified of the GENERAL EMERGENCY and of the.
utility's protective action recommendations. The counties' Executive Groups deliberate over the EDL.
Counties simulate notifications to all field agencies.

Protective Action Recommendations derived from the EAL AGI classification include the evacuation of Sub Areas 1, 2, 3, & Lake.
Protective Action Recommendations derived from the EAL HGl classification and release late plateau include the evacuation of Sub Areas 1, 2, 3, 4, 5 & Lake.
Appropriate officials/agencies/media are informed of the change in classification.
The ODH Sample Screening Point at Ledgemont Elementary School, Thompson Twp., will be simulated - it is not participating in this exercise.
State Assessment Team begins continuous monitoring of emergency workers exposure.

A wind direction change from 290 to 360 occurs which should result in the issuance of a revised offsite protective action recommendation (PAR) by the Perry Plant to recommend EVACUATION out to 5 miles 3600 and down wind out to 10 miles (Subareas 1, 2, 3, 5, 6, 7 &
Lake). Sub Area 4 from the original recommendation will remain on recommendation change.
The State Assessment Team receives a new protective action recommendation from PNPP.
The 3 counties receive the new protective action recommendation from PNPP. They discuss the new PAR with the State and arrive at a second Protective Action Decision.

2008 Evaluated Exercise SCENARIO TIMLINE Medical Services (MS-i) Drill Scenario OVERVIEW The Lake County Emergency Management Agency's medical services drill will be conducted in conjunction with the Perry Nuclear Power Plant on October 7, 2008. The plant will conduct a full-scale exercise that will be evaluated by the NRC. As part of that exercise a "mini-scenario" involving a contaminated and injured victim has been developed (refer to Section 9.0 of the Exercise Manual, Plant Mini-Scenario No. 21).
This scenario involves the response of the Painesville Township Fire Department to the Perry Plant where they will demonstrate handling of a contaminated and injured victim. The Fire Department ambulance crew will contact the Lake West Hospital (Willoughby) and transport the patient to the hospital. No sirens or emergency lights will be used and traffic laws for normal commercial traffic will be followed.
CRITERION 6.d.1 The facility/ORO has the appropriate space, adequate resources, and trained personnel to provide transport, monitoring, decontamination, and medical services to contaminated injured individuals.

2008 Evaluated Exercise SCENARIO TIMLINE The Perry Plant offers its Radiation Protection resources to the Lake Hospital System for any radiological accident response for which the hospital desires such support. Termination of Drill Play: When the patient has been monitored, decontaminated, treated, and ready for movement to regular treatment areas of the hospital, the exercise controller will terminate the drill, with the agreement of the evaluator.

50 Location: Fuel Handling Building 620' While performing decontamination and housekeeping activities in the Fuel handling Building a worker receives a partial thickness laceration to the right forearm while exiting the Bridge Crane.
- 1.
Upon completion of decontamination and housekeeping activities on the Bridge Crane in the Fuel Handling Building, two workers are exiting the crane.
- 2.
While descending the ladder on the North side of the bridge one of the workers lacerates their right forearm on an exposed bolt.
NOTE:
The right sleeve of the PC's is torn and skin contamination is 3000 ccpm/100cm2 as noted on Attachment 2.
- 3.
The non-injured worker is able to assist the injured person to a location outside the FHB and reports the accident to the Control Room -0755. The Secondary Alarm Station (SAS) will tone out the First Aid Team (FAT).
- 4.
Per EPI-B4, the SAS should perform the following: Tone out the FAT and verify that FAT members acknowledge the announcement, Notify the Shift Manager, Security Shift Supervisor (SSS), and Site Dispensary.
Notify the Radiologically Controlled Area (RCA) Control Point to verify that a RadiationProtection Technician (RPT) has been dispatched to the injury scene.

An RPT arrives at the Intermediate Building and observes the injured person sitting with a bloodstain on the right forearm of his modesty garment.
Note: Responding RPT notifies Control Room of injured man

The RPT frisks the right forearm and determines that 3000 ccpm/100 cm2 is found in the wound area.
- 7.
FAT arrives on-scene and ascertains the condition of the victim and observes the following: A 4" Partial thickness laceration to the right forearm.

2008 Evaluated Exercise SCENARIO TIMLINE Vital signs: Pulse 86, Skin warm and dry, Pupils equal and reactive, B/P 170/110 and respirations 32 and regular Clothing should be torn or cut away from the laceration before applying a dressing to prevent contamination from being introduced into the wound.

Based on the nature of the injury the FAT requests off-site medical transport.
Per EPI-B34, the SAS should perform the following: Contact the Lake County Emergency Communications Center by dialing "911", and if known, notify them whether the victim(s) is potentially or actually contaminated.
Notify the Shift Manager, SSS, and Security Officers at the Vehicle Inspection Facility and Vehicle Trap that an ambulance has been requested.

Radiation Protection may attempt to decontaminate the victim based on.
the injury being non-life threatening.
NOTE: RP should be given two attempts at decontamination, any attempt to decontaminate after that will result in pain to the victim's right arm.
- 10.
A RPT will accompany the victim to Lake West Hospital in Willoughby, OH to monitor for potential contamination spread and to assist, if requested, in decontaminating the victim. A second RPT or RP Supervisor will be dispatched to Lake West Hospital per EPI-B34 to assist in monitoring and releasing the ambulance vehicle and personnel, Emergency Room treatment area and equipment, and Emergency Room staff.

Victim decontamination at Lake West Hospital will be successful after Conditions:

The Radiation Protection Technician will determine radiological conditions by performing radiation, contamination and airborne radioactivity surveys. The Radiation and airborne radioactivity surveys are required to meet the Radiation annual HP drill objective.
