Description:

Maintenance discovered P-161C suction piping 90-degree elbow for P-161C continuous SSW hypo pump cracked and leaking into burm area.

2. ApparentDirect Cause: Over-tightening of the PVC elbow caused cracking to occur. It was noted that great difficulty was encountered during removal of the elbow.

NOTE: P-161C SUCTION PIPING 90?ELBOW SHOWED EVIDENCE OF OVERTIGHTENING OF THE PVC ELBOW. DURING ELBOW REPLACEMENT, GREAT DIFFICULTY WAS ENCOUNTERED DURING UNTHREADING OF THE CRACKED PVC ELBOW. A NEW PVC ELBOW WAS INSTALLED AND TIGHTENED SUFFICIENT TO PREVENT LEAKAGE AT THE NPT CONNECTION.

THE 5-FUNCTION VALVE ON THE P-161C DISCHARGE PIPING WAS REPLACED WITH A SINGLE-FLTNCTION AIR BLEED VALVE DUE TO A CRACKED INLET NIPPLE CAUSED BY INSUFFICIENT PIPING SUPPORT AND PULSE VIBRATION FROM THE POSITIVE DISPLACEMENT PUMP.
