Steve Loehlein Response: On-going discussions are being held with plant management. The management oversight continually challenges the root cause team with the same interests. The root cause team is comparing the wastage information between nozzle #3 and nozzle #2, however no specific actions are being taken or planned at this time. The team is also concerned with balancing retrieval of all the available data is being collected while ensuring data is not destroyed and no damage to the reactor head occurrs. Also the increased dose associated with the second nozzle removal must be considered as part of the scope.

The technique uses a softened acetate film, pressed unto the surface of the cavity which is removed and scanned by an electron scanning microscope.

Should we use scanning electron microscope film to take impressions of the nozzle 2 and cavity areas. Per AIT this may help distinguish between impingement and corrosion NRC - Me1 Holmberg Me1 Holmberg's request is to be provided the results of any step of the inspection and sampling plan. 2. Decontamination and area cleanup. Use only a clean vacuum cleaner, no liquids or wiping (save the debris that is vacuumed as samples).

NRC - Me1 Holmberg 3/15/2002 Question The root cause has considered this Closed No technique and as similarly noted in Root Cause CRAIT-0006, the team feels that N/A Steve Fyfitch videotaped information, followed by laser and mechanical measurements and subsequent dential impression(s) will adequately provide the necessary data for assessment of the conditions.

An integrated plan for inspection and evaluation of the nozzle #3 location has been formulated. This will be provided to the AIT for review. NRR has already reviewed and approved the plan.

Results are currently being made available to the root cause team.

2) 2 bags of cloth (weld blankets) and 2 bags of insulation (previously removed from the work platform) were stored in a black barrel. It is located in the locked hi-radiation area north of the reactor headstand on the 623' elevation. All bags are labeled 3-1 3-02 @ 1930 prior to data gathering.

Action Plan Results IAW WO 02-001917-000 During the entire video taping of Nozzle 3, there appeared to be no cavities or divots that still contained caked in boron deposits. Loose boron and miscellaneous debris from the insulation removal process, that was not vacuumed out when step 2 of the QAISP was performed, can be seen in the areas where the sides of the cavity intersect the remaining J-groove weld.

Thus there was no boron to characterize. Also, during the entire video review of the nozzle #3 cavity, there were no cavity surface irregularities observed that would be indicative of steam cutting. All the surface irregularities were indicative of what you would expect from metal that has been eaten away.

The sides of nozzle #3 cavern were relatively clean and the white areas seen in the earlier video were not boron, but metallic color. The cavern appears to go all the way back to nozzle #11.

Note: If the above examination shows that the areas of wastage on the top and bottom of the vessel head are not vertically aligned, the Root Cause Team should be notified immediately to determine if further examination is required.

000 Mark Mcbughlin Performed laser profile utilizing FTl's equipment around nozzle #3 from the top of the service structure. The data was sent to Lynchburg via the T1 line. A model was generated and copies of the model were downloaded to Lynchburg design engineering for use in finite element modeling conducted in Lynchburg. The model data has also been supplied to nozzle # 3 engineering team on site at Davis-Besse.

The void on the downhill side of Nozzle #3 extends approximately 6 inches towards Nozzle #11 and is approximately 5 inches wide at the widest part (a triangular shape). At the back of the void, there is some undercutting that is about 1 112 inches tall and approximately 2 inches from nozzle #11. There is approximately 8 inches between Nozzle #3 and Nozzle

  1. 11. The walls of the cavity are steep with a 75 to 90 degree angle. The void is equivalent to 40 pounds of carbon steel.

The axial flaws were about 3 inches long with approximately 1.2 inches above the weld.

3/19/2002 Clarification NRC Inspector, Jim Davis discussed his Open Yes observation with Prasoon Goyal, Davis Fyfitch, Framatone Engineering. The Prasoon Goyal annealing temperature has an affect on susceptibility of stress corrosion cracking. Heat #M3935 appears to be mostly affected as shown by nozzle leaking data in the Framatone CRDM Nozzle History report. However, the annealing temperature was determined to be consistently maintained above the minimum manufacturing requirement (1600 degrees F) during the fabrication process. Although it is valid that a lower temperature causes a higher susceptibility to stress cracking, the temperature is considered one of many factors and in of itself does not pose a short term concern. Nozzle #4 would be most susceptible to further cracking and should be monitored and assessed if long term usage is required.

Boric acid deposits were generally cleaned up the best that could be accomplished from the top of the Service Structure.

Later discussion with Chuck Daft, mike Sheperd and Pete senuik occurred to discuss the procedure and code requirements. The procedure that applies is DB-PF-03010. For 12RF0, the code applicable wassection XI, 1986 version. The ASME code applicable now is 1995 with 1996 addenda.
