Therefore, in accordance with the requirements of ASME Code Case N-532, the Owners Activity Reports (Form OAR-I) for Surry Unit 2 refueling outages S2R17, S2R18 and S2R19 are provided in Attachment 1. Surry Unit 2 extended the end of the third interval by one year as allowed by the ASME Section XI Code to complete the Ten-Year Reactor Vessel Examination and various other inservice inspection exams.

This information completes the reporting requirements for the third period of the third ten-year inservice inspection (ISI) interval for Surry Unit 2.

Also, Surry Unit 2 simultaneously entered the fourth ten-year IS1 interval on May 10, 2004. Therefore, the Form OAR-I, completed through the duration of the S2R19 refueling outage as allowed by Code Case N-532-1 for the fourth interval IS1 exams, is provided in Attachment 2.

Note 3: The reactor vessel head was replaced during this refueling outage. All new welds received preservice examination.

Note 4: The Class 1 leakage test is required to be performed every refueling outage and was performed on 11/11/03 during this report period. The numbers and percentages represent the total number of refueling outages that occurred during the 3rd Interval.

Note 5: The numbers and percentages represent the total number of periods over the inspection interval.

Note 6: The Risk Informed Program became effective during the 3rd Period. Numbers are for 3rd Period only.

VT-2s are completed in accordance with the System Pressure Test Program.

Note 7: The class 2 pump casing weld requirement is being reevaluated during the R19 RFO per Dominion Letter No.04-337 to the NRC dated June 28, 2004.

Note 8: Relief Request SR-029 was granted 5/24/02 and subsequently implemented. This relief reduced the overall total requirement for this category and skewed completion percentages.

Note 9: Code Case N-509 was implemented, which skewed completion percentages. The numbers and percentages do not include tally of Class 3 pressure tests which are scheduled to be performed by the end of each period. Numbers and percentages do not reflect VT-2 requirements carried by the Pressure Test Program.
