Pursuant to Technical Specification 6.9.1.5.a, Enclosures 1, 2, and 3 are submitted for Salem Unit 1, Salem Unit 2, and Hope Creek, respectively. These enclosures contain 2004 data on the number of station, utility, and other personnel receiving exposures greater than 100 mrem/year and the collective exposures according to work and job function for each unit. provides information pursuant to the requirements of Technical Specification 6.9.1.5.b of Appendix A to Facility Operating Licenses No. DPR-70 and DPR-75. The information pertains to the Salem Unit 1 and Unit 2 steam generator tube inspections completed in 2004.

Pursuant to the requirements of Technical Specification 6.9.1.5.b of Appendix A to Facility Operating License No. NPF-57, the following information is provided concerning the Hope Creek main steamline safety/relief valves (SRVs). During 2004, the SRVs were not challenged by any overpressurization events or transients that would have required the valves to respond.

Pursuant to the requirements of Technical Specification 6.9.1.5.c of Appendix A to Facility Operating Licenses No. DPR-70 and DPR-75, the following information is provided concerning specific activity analyses in which the primary coolant exceeded the limits of Technical Specification 3.4.8. During 2004 there were no events in which

Document Control Desk LR-N05-01 04 Salem Unit I and Unit 2 2004 Steam Generator Tube ISI Report In the spring of 2004 PSEG Nuclear conducted eddfl current examinations on the Salem Unit 1 Steam Generators (SG) during the 16 Refueling Outage (1R16).

No inspections were performed on Salem Unit 2 during 2004. The next scheduled outage for Unit 2 is in April 2005; therefore, this report only addresses Salem Unit 1.

Overview In 1996 Salem Unit I replaced the original steam generators with those from the canceled Seabrook-2 plant. SG replacement was completed in 1998. Salem Unit 1 is a four loop Westinghouse Pressurized Water Reactor (PWR) with Model F SGs that incorporate thermally treated Inconel 600 U-tubes (ASME SB-1 63, Code Case 1484-2). The tube bundle consists of 5,626 tubes with an outside diameter (OD) of 0.688 inches and a nominal tube wall thickness of 0.040 inches.

During SG fabrication, the tubes were hydraulically expanded over the full tubesheet depth and the first ten tube rows were thermally stress relieved after u-bend formation. The tube support plates are fabricated from 405 stainless steel (SS) and have broached uquatrefoil" tube holes. Tube support plates 1, 2 and 3 are 0.75 inches thick and plates 4, 5, 6 and 7 are 1.12 inches thick. A 0.75-inch thick flow distribution baffle (FDB) plate, also 405SS, between the secondary face of the tubesheet and the first tube support plate, provides top-of-tubesheet (TTS) sludge pile control. Tube support within the U-bend region is provided by three v-shaped chrome-plated Inconel 600 antivibration bars (AVB's) 0.275 inches in width.

The steam generator primary side serves as part of the Reactor Coolant pressure boundary for containing the reactor coolant and/or emergency core cooling flow and for limiting radiation release to the Reactor Containment (by limiting coolant leakage) during all plant operating conditions. In addition, the steam generators serve as part of the Reactor Coolant pressure boundary to preclude leakage and radiation release and provide separation between the radioactive primary side and the non-radioactive secondary side/system, Main Steam system, and the outside atmosphere during all plant operating conditions.

Examinations Performed (Scope)

The base eddy current scope was delineated in the Engineering Evaluation No.

S-1 -RC-MEE-1 824 u1 R1 6 Steam Generator Tubing Degradation Assessment".

This document identified the active and potential degradation mechanisms affecting the tubing in the Salem Unit 1 Steam Generators, identified the Page 1 of 40

Document Control Desk LR-N05-1 04 inspection plan and examination techniques to be used, and provided structural limits for each damage mechanism for assessing tube integrity requirements.

The eddy current examination scope met Salem Unit 1 Technical Specification 3/4.4.5, "Steam Generators" Surveillance Requirements.

A summary or the scope performed is outlined below:

  • With the exception of those row 1 and row 2 U-bends inspected under rotating coil inspection program, a full-length bobbin coil inspection was performed on 100% of the in-service tubes in each steam generator.
  • 20% sample rotating coil (+ Point) exam of the Row I and Row 2 U-bends in each SG.
  • Approximately 50% rotating coil (+Point) exam of the Hot Leg (HL) Top of Tubesheet (TTS) transition regions in each steam generator at an extent of

+3 inches above/-3 inches below.

  • 20% sample rotating coil (+Point) exam of the > 5-volt dented HL Tube Support Plate (TSP) intersections (01 H to 07H) in each SG.
  • 20% sample rotating coil (+Point) exam of the > 5-volt HL free span dings (TSH +0.5 inches to 07H) in each steam generator.
  • 100% rotating coil (+ Point) of > = 2-volt U-bends dings (07H to 07C) in each SG.
  • 100% rotating coil (+Point) of > = 2-volt dented Anti-Vibration Bar (AVB's) locations in each SG.
  • Rotating coil (+ Point) of pluggable AVB wear indications in each steam generator.
  • In each SG, performed a rotating coil (+ Point) of historical tubesheet expansion anomalies.

Examination Results and Technical Specification Classification Consistent with the requirements specified in NEI 97-06, Steam Generator Program Guidelines, the Unit 1 steam generators met the structural integrity, accident induced leakage and operational leakage performance criteria specified in site procedure SC.SG-AP.ZZ-0001(Q), Steam Generator Management Program for 1 RI 6. The following table summarizes the number of tubes repaired in each steam generator by degradation mechanism. Framatome designed Alloy 690 rolled plugs were used for these repairs. The table also provides the total number and percentage of tubes repaired to date and cumulative steam generator tube plugging levels:

Anti-vibration Bar (AVB) Wear AVB wear indications are plugged if bobbin indicates a depth > 40% 1W. Tubes with degradation less than 40% TW may be left in service or removed from service depending on the observed growth rate of the degradation. A population of bobbin coil AVB wear indications were inspected with rotating coil that verified the reported indications displayed a volumetric response and were not crack like.

A total of twenty-eight tubes were repaired for this damage mechanism. The table below lists the tubes plugged for AVB wear during 1 R16:

Intergranular Attack I Stress Corrosion Cracking (IGAISCC)

During 1 R16, rotating coil + Point inspections were performed in each steam generator at the hot leg top of tubesheet expansion transitions, at dented locations (freespan straight section, u-bends and at structures) and within row one and row two u-bends. The inspections performed are summarized below:

  • 20% sample rotating coil (+Point) exam of the Row 1 and Row 2 U-bends in each SG.
  • Approximately 50% rotating coil (+ Point) exam of the Hot Leg (HL) Top of Tubesheet (iTS) transition regions in each steam generator at an extent of

+3 inches above/-3 inches below.

  • 20% sample rotating coil (+ Point) exam of the > 5-volt dented HL Tube Support Plate (TSP) intersections (01 H to 07H) in each SG.
  • 20% sample rotating coil (4-Point) exam of the > 5-volt HL free span dings (TSH +0.5 inches to 07H) in each steam generator.
  • 100% rotating coil (+Point) of > = 2-volt U-bends dings (07H to 07C) in each SG.
  • 100% rotating coil (+Point) of > = 2-volt dented Anti-Vibration Bar (AVB's) locations in each SG.
  • In each SG, performed a rotating coil (+ Point) of historical tubesheet expansion anomalies.

There was no IGAJSCC reported during 1 R16. These findings are consistent with that seen in the industry for similar design steam generators with low operating time.

Document Control Desk LR-N05-01 04 o OPERATING EXPERIENCE - ODSCC at the Tube Support Plate (TSP)

Intersections Based on the detection of TSP ODSCC in tubes identified as having higher residual stress at both Seabrook Unit I and Braidwood Unit 2, PSEG Nuclear screened their 1 R1 5 bobbin coil data for tubes in row 11 and higher for signals representative of higher residual stress using a method similar to the Westinghouse provided guidance for measuring the voltage offset between the straight section of tubing and the U-bend (two measurements per tube).

During 1 R1 6, over and above the standard analysis process the suspect population (approximately 185 tubes) was independently reviewed by the site eddy current Level ll. No indications of stress and/or corrosion related degradation was found in any of the tubes inspected. Two tubes in SG 13 were identified as having noticeably smaller voltage offsets than the remaining tubes reviewed. Although no degradation was detected, these tubes were preventatively tube plugged (PTP) during 1 R16. These tubes are listed in the table below:

A total of three tubes were plugged due to wear indications (SVI) associated with a loose part that was removed from the cold leg tubesheet of II SG. The tubes repaired are listed in the table below:
