ANNUAL ASSESSMENT LETTER - LASALLE NUCLEAR POWER STATION (REPORT 05000373/200501; 05000374/200501)
Dear Mr. Crane:
On February 9, 2005, the NRC staff completed its end-of-cycle plant performance assessment of the LaSalle County Station. The end-of-cycle review for the LaSalle County Station involved the participation of the reactor technical divisions in evaluating performance indicators (PIs) for the most recent quarter and inspection results for the period from January 1 through December 31, 2004. The purpose of this letter is to inform you of our assessment of your safety performance during this period and our plans for future inspections at your facility so that you will have an opportunity to prepare for these inspections and to inform us of any planned inspections that may conflict with your plant activities.
This performance review and enclosed inspection plan do not include physical protection information. A separate letter designated and marked as Exempt from Public Disclosure in accordance with 10 CFR 2.390 will include the physical protection review and resultant inspection plan.
Overall, LaSalle Nuclear Power Station operated in a manner that preserved public health and safety and fully met all cornerstone objectives. Plant performance for the most recent quarter, as well as for the first three quarters of the assessment cycle, was within the Licensee Response Column of the NRCs Action Matrix, based on all inspection findings being classified as having very low safety significance (Green) and all PIs indicating performance at a level requiring no additional NRC oversight (Green). Therefore, we plan to conduct only reactor oversight process (ROP) baseline inspections at your facility through September 30, 2006.
Additionally, the substantive cross-cutting issue in the area of human performance, identified during the mid-cycle assessment, continued as a regulatory concern during the end-of-cycle assessment. Plant performance in this cross-cutting area was assessed by expending efforts to review problem identification and resolution (PI&R) issues during routine baseline inspections.
Our inspection staff identified ten GREEN findings/violations where human performance was not adequate during the assessment period. Seven of these were previously evaluated as part of the twelve findings considered during the mid-cycle assessment, while three are new. Of

(two mitigating systems cornerstone, and one barrier integrity cornerstone), and the remaining seven are in the radiation safety SPA (all in the occupational radiation safety cornerstone).
In our mid-cycle assessment letter dated August 30, 2004, we advised you of our identification of a substantive human performance cross-cutting issue. In response to the identification of human performance deficiencies, you have developed a comprehensive human performance plan and have begun execution of that plan. Each of the individual performance errors was entered into your corrective action program and you have completed root cause analyses on a department by department basis for certain human performance issues. However, the effectiveness of these corrective actions is still indeterminate, as evidenced by the continued occurrence of human performance events/issues at LaSalle County Station.
Because of continued concerns with human performance at LaSalle County Station, we request that you discuss this substantive cross-cutting issue during the end-of-cycle meeting and that you provide a written response to the NRC, within 60 days of the date of this letter, detailing corrective actions taken, the effectiveness of implemented corrective actions, and any additional corrective actions needed as a result of the continued occurrence of issues having human performance as a significant causal factor or contributing cause.
The enclosed inspection plan details the inspections, less those related to physical protection scheduled through September 30, 2006. The biennial PI&R inspection is scheduled for October 2005. The inspection plan is provided to minimize the resource impact on your staff and to allow for scheduling conflicts and personnel availability to be resolved in advance of inspector arrival onsite. Routine resident inspections are not listed due to their ongoing and continuous nature. The inspections in the last 9 months of the inspection plan are tentative and may be revised at the mid-cycle review meeting.
In accordance with 10 CFR 2.390 of the NRCs Rules of Practice, a copy of this letter and its enclosure will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRC's document system (ADAMS).
