MID-CYCLE PERFORMANCE REVIEW AND INSPECTION PLAN -

LASALLE COUNTY STATION

Dear Mr. Crane:

On August 11, 2004, the NRC staff completed its mid-cycle plant performance assessment of LaSalle County Station. The mid-cycle review for LaSalle County Station involved the participation of all technical divisions in evaluating performance for the first half of the calendar year 2004 assessment cycle. The process included a review of performance indicators (PIs) for the most recent quarter and inspection results over the previous 12 months. The purpose of this letter is to inform you of our assessment of your safety performance during this period and our plans for future inspections at your facility so that you will have an opportunity to prepare for these inspections and to inform us of any planned inspections that may conflict with your plant activities.

Plant performance for the most recent quarter was within the Licensee Response Column of the NRCs Action Matrix, based on all inspection findings being classified as having very low safety significance (Green) and all PIs indicating performance at a level requiring no additional NRC oversight (Green). Therefore, we plan to conduct only reactor oversight process (ROP) baseline inspections at your facility through March 31, 2006.

During this assessment period, the staff identified a substantive cross-cutting issue in the area of human performance. This human performance cross-cutting issue was identified by assessing plant performance utilizing Inspection Procedures (IP) 71152 to review problem identification and resolution (PI&R) issues during routine baseline inspections and during the biennial PI&R inspection.

Over the course of the assessment period, the inspectors identified twelve GREEN findings/violations where human performance was not adequate. Six of these were in the reactor safety strategic performance area (SPA) (two initiating events and four mitigating

Crane systems cornerstones), four were in the radiation safety SPA (all in the occupational radiation safety cornerstone) and two more were unresolved issues in the radiation safety SPA (occupational radiation safety cornerstone). Specifically, the findings/violations were attributed to inadequate human performance in manipulation of plant equipment outside of the normal work control processes and without permission from the control room (three), failing to comply with procedural requirements (three), failure to comply with contaminated and high radiation area posting requirements (four), and failure to post radiation or high radiation signs on scaffolding platforms (two). In addition to the twelve findings, another eighteen additional minor issues were identified with human performance as the primary or contributing cause. All of these issues were in the reactor safety SPA (five in the initiating events, nine in the mitigating systems, and four more in the barrier integrity cornerstones).

In response to the identification of human performance deficiencies, you have developed a comprehensive human performance plan and have begun execution of that plan. Each of the individual human performance errors discussed above was entered into your corrective action program and you have completed root cause analyses on a department by department basis for certain human performance issues.

We plan to conduct reactor oversight process baseline inspections at your facility through March 31, 2005. In addition, we plan to perform a focused inspection using IP 71152, Identification and Resolution of Problems. This inspection will select human performance issues for a more in-depth review. This includes use of the inspection guidance contained in IP 95001, Inspection For One Or Two White Inputs In A Strategic Performance Area, as an aid in assessing the adequacy of your root cause analyses of human performance issues at your facility through February 28, 2005.

The enclosed inspection plan details the inspections scheduled through March 31, 2006. The inspection plan was provided to minimize the resource impact on your staff and to allow for scheduling conflicts and personnel availability to be resolved in advance of inspector arrival onsite. Routine resident inspections are not listed due to their ongoing and continuous nature.

The inspections in the last 12 months of the inspection plan are tentative and may be revised at the end-of-cycle review meeting.

In accordance with 10 CFR 2.790 of the NRCs Rules of Practice, a copy of this letter and its enclosure will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRC's document system (ADAMS).
